Post-Retinal Laser Surgery: Visual Disturbances and Symptoms - Ophthalmology

Postoperative retinal issues

Hello, Doctor: About a year and a half ago, I underwent a dilated retinal examination and discovered a retinal tear in one eye.
I subsequently had several sessions of laser photocoagulation.
Prior to the retinal examination, I did not notice any abnormalities; I went for the check-up because several family members have retinal diseases (middle-aged and elderly).
However, a few days after completing all the laser treatments, I accidentally noticed a wavy, ripple-like movement in the corner of my visual field.
At that time, I thought that since I had just been examined and treated with laser, everything should be fine.
To this day, there have been no significant changes, and the ripples still persist.
Recently, I have also felt an increase in floaters in that eye, which has led me to start connecting the floaters with the ripples.
I found online that ripples may be part of the aging process of the vitreous body, which can vary in duration.
My question for you, Doctor, is whether the side effects of retinal laser treatment include the appearance of ripples and floaters? Thank you! Wishing you all the best.

Hello: Water waves and floaters are both phenomena related to vitreous degeneration, and retinal tears are usually caused by the pulling effect during vitreous degeneration.
Therefore, they are not necessarily related to laser treatment.
Laser procedures typically do not cause these phenomena.

After undergoing retinal laser surgery, it is not uncommon for patients to experience various visual disturbances, including the perception of wavy lines (often referred to as "metamorphopsia") and an increase in floaters (known as "myodesopsia"). These symptoms can be concerning, especially for individuals who have a family history of retinal diseases, as you mentioned.

Understanding Visual Disturbances Post-Surgery
1. Wavy Lines (Metamorphopsia): This phenomenon can occur due to changes in the retina or the underlying structures following laser treatment. The laser procedure aims to create a scar around the retinal tear or hole, which can sometimes lead to alterations in how light is processed by the retina. If the retina is still healing or if there are changes in the vitreous gel that fills the eye, it can result in distorted vision. This is not necessarily a direct complication of the laser surgery but rather a result of the ongoing changes in the eye's anatomy.

2. Increased Floaters: The appearance of new floaters after laser surgery can also be linked to the natural aging process of the vitreous gel. As we age, the vitreous can become more liquefied and may pull away from the retina, leading to the perception of floaters. However, in the context of recent laser surgery, it is essential to monitor these symptoms closely. An increase in floaters, especially if accompanied by flashes of light or a sudden change in vision, could indicate a potential complication such as a retinal detachment.

Potential Causes of Symptoms
- Vitreous Changes: After laser surgery, the vitreous gel may undergo changes that can lead to the perception of floaters and wavy lines. This is often a benign process but should be monitored.

- Retinal Healing: The healing process of the retina post-laser treatment can also contribute to visual disturbances. As the retina heals, it may not function optimally, leading to temporary visual distortions.

- Underlying Conditions: If there were pre-existing conditions that prompted the laser treatment, such as lattice degeneration or retinal tears, these could also play a role in the symptoms you are experiencing.

Recommendations
1. Regular Follow-Up: It is crucial to maintain regular follow-up appointments with your ophthalmologist. They can monitor the health of your retina and vitreous and assess any changes in your symptoms.

2. Visual Symptoms Monitoring: Keep a record of your symptoms, including when they occur and any changes in intensity. This information can be valuable for your doctor in determining the best course of action.

3. Avoid Strain: If you notice that certain activities exacerbate your symptoms, such as prolonged screen time or reading, consider taking breaks and reducing eye strain.

4. Seek Immediate Care if Necessary: If you experience sudden changes in vision, such as a significant increase in floaters, flashes of light, or a shadow in your peripheral vision, seek immediate medical attention. These could be signs of a more serious condition.

Conclusion
In summary, while the symptoms of wavy lines and increased floaters can be concerning, they are not uncommon after retinal laser surgery. These disturbances may be related to the healing process or changes in the vitreous gel. Regular monitoring and communication with your eye care provider are essential to ensure that your symptoms are appropriately managed and to rule out any serious complications. Always prioritize your eye health and do not hesitate to reach out to your ophthalmologist with any concerns.
