Persistent bleeding issues?

Hello, Doctor.
I am a patient with rheumatoid arthritis.
I underwent a simple fistula incision surgery a month ago, but I am currently experiencing bleeding during bowel movements, even having blood in my stool.
Is this normal? I had a ligation procedure for three internal hemorrhoids two months ago, where the doctor used rubber bands.
I am concerned that the internal hemorrhoids were not completely treated or have recurred.
Would taking hemostatic medication from the pharmacy be helpful? Additionally, do I need to undergo another colonoscopy? I had one a year ago, which did not reveal any abnormalities.
Thank you, Doctor!

Nini, 30~39 year old female. Ask Date: 2021/03/27

Dr. Ke Fangxu reply Surgery

It is normal to experience slight bleeding and discharge within a month after fistula surgery.
If you are concerned, you can ask the surgeon for a follow-up examination.
Hemorrhoid ligation is not a curative procedure, and recurrence of symptoms is a common occurrence.
If you had a colonoscopy a year ago with no abnormal findings, it is generally recommended to have a follow-up examination in 3 to 5 years.

Persistent bleeding after surgery, particularly following procedures like fistula incision or hemorrhoid ligation, can be concerning and warrants careful evaluation. In your case, experiencing bleeding after a simple fistula surgery, especially if it includes blood in your stool, is not typical and should be addressed promptly.

Firstly, it’s essential to understand that some bleeding can occur post-operatively, particularly in the first few days to weeks after surgery. However, if you are experiencing ongoing bleeding or blood in your stool, this could indicate several potential issues that need to be evaluated by a healthcare professional.

1. Possible Causes of Bleeding:
- Incomplete Healing: After a fistula surgery, the surgical site may not have healed completely, leading to persistent bleeding. This is particularly true if there is any tension or strain on the area during bowel movements.

- Hemorrhoid Issues: Since you had a ligation procedure for hemorrhoids two months prior, it’s possible that the hemorrhoids were not fully resolved or have recurred. Hemorrhoids can cause bleeding, especially during bowel movements.

- Other Gastrointestinal Issues: Blood in the stool can also indicate other gastrointestinal problems, such as diverticulosis, inflammatory bowel disease, or even colorectal polyps or cancer, although these are less likely given your previous normal colonoscopy.

2. Management and Next Steps:
- Consult Your Surgeon or Gastroenterologist: It’s crucial to reach out to your healthcare provider to discuss your symptoms. They may recommend a physical examination and possibly a repeat colonoscopy to assess the situation further, especially since you have a history of gastrointestinal issues.

- Avoid Self-Medication: While over-the-counter medications like anti-hemorrhoidal creams or oral medications may provide temporary relief, they are not a substitute for professional medical advice. Taking hemostatic agents without a clear diagnosis could mask underlying issues.

- Dietary Considerations: Ensure you are consuming a high-fiber diet to promote regular bowel movements and reduce straining, which can exacerbate bleeding. Staying hydrated is also essential.

3. When to Seek Immediate Care:
- If you experience significant amounts of blood, severe pain, or any signs of infection (such as fever or increased swelling), seek immediate medical attention. These could be signs of complications that require urgent intervention.

4. Follow-Up Care:
- Regular follow-up appointments with your healthcare provider are essential to monitor your recovery and address any ongoing issues. They can provide tailored advice based on your specific situation and medical history.

In summary, while some bleeding can be expected after surgery, persistent bleeding, especially with blood in the stool, is not normal and should be evaluated by a healthcare professional. It’s essential to communicate openly with your doctor about your symptoms and concerns to ensure appropriate management and care.
