Post-thyroidectomy
Hello, I underwent a total thyroidectomy for malignant thyroid tumor in December 2020, diagnosed as stage I papillary thyroid carcinoma.
I am scheduled to receive radioactive iodine treatment at a dose of 100 mCi at the end of April 2021.
Is this dose considered severe? Also, will total thyroidectomy affect my menstrual cycle? My menstrual cycle has been irregular since the third month post-surgery.

After a total thyroidectomy for papillary thyroid carcinoma, particularly in early-stage cases like yours (pT1bN0), the subsequent treatment with radioactive iodine (I-131) is a common and effective approach. Your planned dose of 100 mCi is considered a standard therapeutic dose for post-surgical treatment of thyroid cancer, especially when there is a risk of residual disease or metastasis. While this dose is significant, it is not uncommon in the management of thyroid cancer, and it is intended to ablate any remaining thyroid tissue that may harbor cancer cells.
Regarding your concerns about menstrual irregularities following thyroid surgery, it is important to understand that the thyroid gland plays a crucial role in regulating various bodily functions, including metabolism and hormonal balance. Thyroid hormones significantly influence the menstrual cycle, and any disruption in their levels can lead to menstrual irregularities. After a total thyroidectomy, patients typically require lifelong thyroid hormone replacement therapy (usually levothyroxine) to maintain normal hormone levels. If the dosage of your replacement therapy is not adequately adjusted post-surgery, it can lead to hypothyroidism (low thyroid hormone levels), which may cause menstrual irregularities, including heavier, lighter, or missed periods.
In your case, experiencing menstrual irregularities three months post-surgery could be related to several factors:
1. Thyroid Hormone Levels: If your thyroid hormone levels are not yet stabilized, this could directly affect your menstrual cycle. It is essential to have your TSH (Thyroid Stimulating Hormone) and Free T4 levels monitored regularly after surgery and during I-131 treatment to ensure they are within the normal range.
2. I-131 Treatment: The radioactive iodine treatment itself is not typically associated with direct effects on menstrual cycles. However, the stress of the surgery and the subsequent treatment can impact your overall hormonal balance and may contribute to irregularities.
3. Psychological Factors: The emotional and psychological stress of dealing with a cancer diagnosis and treatment can also affect menstrual cycles. Stress can lead to hormonal fluctuations that may disrupt your regular cycle.
4. Other Health Factors: Other underlying health issues unrelated to your thyroid condition, such as polycystic ovary syndrome (PCOS) or other hormonal imbalances, could also be contributing to your menstrual irregularities.
It is advisable to discuss these changes with your healthcare provider. They may recommend blood tests to check your thyroid hormone levels and assess whether your current dosage of levothyroxine is appropriate. If necessary, they can adjust your medication to help stabilize your hormone levels and potentially restore your menstrual cycle.
In summary, while a dose of 100 mCi of I-131 is standard for your situation, the impact of thyroid surgery and subsequent treatment on your menstrual cycle can be significant. Regular monitoring and communication with your healthcare team are crucial to managing your thyroid hormone levels and addressing any concerns about menstrual irregularities.
