What are the surgical treatments and costs associated with arthroscopy?
Hello, doctor.
Last July, while climbing a rock wall, my knee suddenly made a loud noise and felt loose and painful.
I later went to a medical center where I was diagnosed with an "anterior cruciate ligament (ACL) sprain" after an X-ray.
I asked the doctor if my meniscus was torn, but he replied that the meniscus is not that fragile.
He only prescribed anti-inflammatory medication and sent me home to rest.
However, by September, I felt that my condition had not improved much, so I returned to the same hospital to see another doctor.
This doctor, after listening to my symptoms, simply administered a hyaluronic acid injection and prescribed anti-inflammatory medication before sending me home.
Although the pain improved significantly after the injection, I still felt that I had not fully recovered.
I endured until October, when I sought treatment at an orthopedic clinic.
The clinic doctor absurdly claimed that my ACL was not injured and stated that the meniscus is not easily damaged.
After taking another X-ray, he discovered abnormal wear on the cartilage of my right knee (the affected area), but he still just prescribed anti-inflammatory medication.
Last month, I could no longer tolerate the pain, so I sought another medical center for an MRI to understand the truth.
The results confirmed a "meniscus tear." Currently, my symptoms include an inability to bend or squat my knee, and I experience sharp pain and inflammation in the lower left area of my right knee.
When I asked the doctor, he said I would need to undergo "arthroscopic surgery," which would cost around 100,000 NTD (out-of-pocket).
He also mentioned that if additional equipment was needed, it could cost an extra 20,000 to 30,000 NTD.
I don't understand why such an injury requires such a high surgical fee.
Isn't the cost for a knee replacement similar? I'm not sure if I misunderstood or if the doctor misspoke.
Where else can I inquire about this type of surgery? I kindly request your clarification.
Thank you.

Dr. Chen Bochen reply Orthopedics
Arthroscopic surgery itself is costly under health insurance primarily due to the necessary consumables: the electrocautery used in the arthroscope, disposable tubing, sutures for meniscus repair, PRP, etc.
You can inquire with several providers; you might find cheaper options, but you may end up using previously used electrocautery, reused tubing, or traditional sutures.
There are always two sides to every issue: if you want safety and affordability, you might sacrifice effectiveness; if you want effectiveness and safety, it may require spending more; if you want affordability while ensuring safety, it could be a scam.
I wish you the best in finding a suitable physician and a speedy recovery.

Arthroscopic surgery for knee injuries, particularly for conditions like a torn meniscus, can indeed be a significant financial commitment. The cost you mentioned, approximately 100,000 TWD (around 3,300 USD), is not uncommon, especially when considering the various factors that contribute to the overall expense of the procedure.
Firstly, it's important to understand what arthroscopic surgery entails. This minimally invasive procedure allows surgeons to diagnose and treat knee problems through small incisions using a camera (arthroscope) and specialized instruments. While the surgery itself may seem straightforward, the costs can escalate due to several factors:
1. Surgical Equipment and Supplies: The cost of the surgical instruments, including the arthroscope, specialized tools for repairing the meniscus, and consumables like sutures and saline solutions, can be quite high. These items often need to be disposed of after a single use, which adds to the overall cost. Additionally, if advanced technologies such as PRP (Platelet-Rich Plasma) therapy are used, this can further increase expenses.
2. Facility Fees: The hospital or surgical center where the procedure is performed will charge facility fees. These fees cover the use of the operating room, recovery room, and other hospital services. Depending on the facility, these costs can vary significantly.
3. Anesthesia Costs: Anesthesia is typically required for arthroscopic surgery, and the anesthesiologist's fees can also contribute to the total cost.
4. Surgeon's Fees: The surgeon's experience and reputation can influence their fees. Highly skilled orthopedic surgeons may charge more for their expertise.
5. Post-Operative Care: After the surgery, there may be additional costs for follow-up visits, physical therapy, and any necessary medications. Rehabilitation is crucial for recovery, and physical therapy can add to the overall expense.
Regarding your concern about the costs being comparable to those of a total knee replacement, it's essential to note that while both procedures involve the knee, they are fundamentally different in terms of complexity and recovery. Total knee replacement is a more invasive procedure that involves replacing the entire knee joint, which typically incurs higher costs due to the complexity of the surgery and the prosthetic components used.
If you're seeking alternative opinions or lower-cost options, consider the following steps:
- Consult Multiple Orthopedic Surgeons: Getting second or third opinions can provide you with different perspectives on your condition and treatment options. Each surgeon may have varying approaches and costs associated with their services.
- Explore Different Facilities: Prices can vary significantly between hospitals and surgical centers. Researching different facilities may help you find a more affordable option.
- Insurance Coverage: Check with your health insurance provider to see what aspects of the surgery may be covered. Some plans may cover a portion of the costs, especially if the procedure is deemed medically necessary.
- Discuss Payment Plans: Many healthcare facilities offer payment plans or financing options to help manage the costs of surgery.
In conclusion, while the costs associated with arthroscopic surgery for a torn meniscus can be high, understanding the various components that contribute to this expense can help you navigate your options. It's crucial to communicate openly with your healthcare providers about your concerns and explore all available avenues for treatment and financial assistance. Your health and recovery should be the priority, and finding the right care is essential for achieving the best possible outcome.
