The scrotum hangs low and causes discomfort by pressing against the testicles when sitting down?

Hello doctor, my scrotum is very long and hangs down, and even during an erection, I still have this issue.
It becomes bothersome during intercourse as it easily sways or hits my testicles during thrusting, causing discomfort.
Additionally, when I sit down, I often experience pressure on my testicles, making it difficult to sit still and I constantly need to change my sitting position.
What could be the cause of this? Thank you!

Hello: The cremaster muscle causes the testicles to ascend and descend, which varies from person to person.
Generally, as one ages, the degree of movement tends to decrease.
Wishing you good health.

Understanding the discomfort associated with a long scrotum can be a multifaceted issue, often involving anatomical, physiological, and sometimes psychological factors. The scrotum is designed to keep the testicles at a temperature slightly lower than the body's core temperature, which is essential for optimal sperm production. However, variations in scrotal length and the associated discomfort can arise from several causes.

Causes of Long Scrotum and Associated Discomfort
1. Anatomical Variations: Just like any other part of the body, the scrotum can vary significantly in size and shape from person to person. A longer scrotum may simply be a normal anatomical variation. However, if this length is accompanied by discomfort, it may be worth investigating further.

2. Temperature Regulation: The scrotum's ability to contract and expand is crucial for temperature regulation. If the scrotum is excessively long, it may not retract adequately during arousal or physical activity, leading to discomfort during sexual intercourse or while sitting.

3. Inguinal Hernia: In some cases, a long scrotum may be associated with an inguinal hernia, where abdominal contents protrude through a weak spot in the abdominal muscles. This can lead to discomfort and should be evaluated by a healthcare professional.

4. Hydrocele: A hydrocele is a fluid-filled sac surrounding a testicle that can cause swelling and discomfort. If the scrotum appears elongated and is accompanied by swelling, this could be a potential cause.

5. Varicocele: This condition involves enlarged veins within the scrotum, which can lead to a feeling of heaviness or discomfort. Varicoceles are often more prominent when standing and can cause discomfort during physical activities.

6. Psychological Factors: Sometimes, discomfort can be exacerbated by anxiety or stress related to sexual performance or body image. This can lead to a heightened awareness of physical sensations, making discomfort feel more pronounced.

Solutions and Recommendations
1. Consult a Healthcare Professional: The first step in addressing your concerns should be to consult a urologist. They can perform a physical examination and possibly imaging studies to rule out conditions like hernias or varicoceles.

2. Supportive Underwear: Wearing supportive underwear can help manage discomfort by providing better support to the scrotum and testicles, reducing excessive movement during physical activities.

3. Positioning: Experimenting with different positions during sexual activity may help minimize discomfort. For instance, positions that allow for less movement of the scrotum may be more comfortable.

4. Sitting Posture: Adjusting your sitting posture can alleviate pressure on the testicles. Consider using a cushion or adjusting the height of your chair to find a more comfortable position.

5. Physical Therapy: In some cases, pelvic floor physical therapy may be beneficial. A trained therapist can help you learn exercises to strengthen the pelvic floor muscles, which may help alleviate discomfort.

6. Surgical Options: If anatomical issues are identified that cause significant discomfort, surgical options may be available. This could involve procedures to correct hernias, varicoceles, or other anatomical concerns.

7. Mental Health Support: If anxiety or stress is contributing to your discomfort, consider speaking with a mental health professional. They can help you develop coping strategies and address any underlying psychological issues.

Conclusion
Experiencing discomfort due to a long scrotum can be distressing, but it is essential to remember that help is available. By consulting with a healthcare professional, you can explore the underlying causes of your discomfort and discuss appropriate treatment options. Remember, your health and comfort are paramount, and addressing these issues can lead to a significant improvement in your quality of life.
