There is a swelling near the surgical wound?

Hello Doctor, on February 18, 2021, I underwent fistula surgery at a medical center affiliated with a medical university in Vietnam.
A total of three fistulas were treated.
Due to the pandemic, I was unable to return to Taiwan for follow-up care.
Postoperatively, the wounds were managed with silver ion dressings and debridement ointment (no sitz baths were recommended by the Vietnamese doctor).
As of today, the wounds have generally healed well.
However, on April 5, I noticed a small abscess near the wound (as shown in the Before image).
My wife accidentally punctured the abscess while dressing the wound and applied medication.
Today, April 8, the previous abscess has shown slight swelling and tenderness (as shown in the After image).
I would like to ask if this indicates a recurrence of the fistula after surgery, or could it be that the Vietnamese doctor did not adequately clean the fistula during the procedure? Post-surgery swelling and the presence of a pus-filled bump near the surgical site can indeed raise concerns about complications, especially following a procedure like fistula surgery. It is essential to understand the potential implications of these symptoms and the necessary steps to address them.

Firstly, the appearance of swelling and pain near the surgical site, particularly after the initial healing period, may indicate an infection or inflammation. Infections can occur post-operatively due to various reasons, including inadequate wound care, the presence of foreign material, or even the body's response to the surgical procedure itself. The fact that you noticed a pus-filled bump suggests that there may be an accumulation of pus, which is typically a sign of infection.
In your case, the timing of the symptoms—occurring several weeks after surgery—could suggest that the infection is either a result of the surgical procedure itself or a complication arising from the healing process. It is also possible that the fistula was not entirely excised or cleaned out during the surgery, leading to residual infection or inflammation.
Regarding your concerns about whether this is a recurrence of the fistula, it is crucial to differentiate between a new infection and a recurrence of the original condition. A recurrence would typically present with similar symptoms to the original fistula, such as drainage or discomfort in the same area. However, the presence of pus and swelling is more indicative of an infection rather than a straightforward recurrence of the fistula.

Given that you are currently in Vietnam and unable to return to Taiwan for follow-up care, it is advisable to seek medical attention from a local healthcare provider. They can perform a thorough examination, possibly including imaging studies or cultures of the pus, to determine the exact nature of the swelling and the best course of action. Treatment may involve antibiotics if an infection is confirmed, and in some cases, further surgical intervention may be necessary to address any residual fistula or to drain any abscess that may have formed.

In the meantime, it is essential to keep the area clean and dry, avoid any further manipulation of the bump, and monitor for any worsening symptoms such as increased redness, warmth, or fever, which could indicate a more severe infection.
In summary, while post-surgical swelling and the presence of a pus-filled bump can be concerning, they do not automatically indicate a failure of the original surgery. However, they do warrant prompt medical evaluation to ensure appropriate management and to prevent further complications. Please consult a healthcare professional as soon as possible to address these symptoms effectively.
