Prostatitis

Hello Doctor: In the past 2 to 3 years, I have experienced episodes of increased sexual desire, leading to multiple instances of masturbation or sexual intercourse (4 to 5 times within 2 to 3 days), followed by episodes of prostatitis (approximately 4 to 5 times a year).
Could this be related to my age? (I don't recall experiencing this during my school years or early career).
The symptoms include a burning sensation in the bladder after urination, but there is no discomfort during urination, so it shouldn't be urethritis, right? Could it be due to a past unsafe sexual encounter that may have led to a gonorrhea or chlamydia infection (as I received treatment without testing at that time)? (I did undergo STI screening after that treatment, and the results were normal).

Additionally, I would like to ask the doctor: although my sexual desire remains strong, I find it difficult to achieve an erection, and when I do, the hardness is insufficient, making it easy to lose rigidity during intercourse or ejaculate with inadequate hardness.
Is this considered a sexual dysfunction? Could it be related to my previous attempts to prolong sexual activity by delaying ejaculation? Or is it simply due to age, as I may not experience the same level of excitement and impulsiveness as when I was younger, and I am now more familiar and experienced with the female body, leading to reduced sexual arousal? Thank you for your responses to my questions, Doctor.

Qian, 30~39 year old female. Ask Date: 2021/04/11

More Info

Prostatitis, particularly chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS), is a common condition that can significantly affect sexual health and overall well-being. Your symptoms and experiences raise several important points that warrant a comprehensive understanding of the condition, its causes, and its implications for sexual health.

Causes of Prostatitis
Prostatitis can arise from various factors, including:
1. Infections: While bacterial infections can lead to acute prostatitis, chronic prostatitis may not always be linked to bacterial causes. In your case, previous episodes of prostatitis following sexual activity could suggest a connection to sexual health practices, including the possibility of sexually transmitted infections (STIs) like gonorrhea or chlamydia. However, since you mentioned that subsequent STI screenings were normal, it may indicate that the initial infection was effectively treated or that the symptoms are not due to an active infection.

2. Pelvic Floor Dysfunction: Chronic pelvic pain can also stem from muscle tension or dysfunction in the pelvic floor, which can be exacerbated by sexual activity, prolonged sitting, or stress. This could explain the discomfort you experience after frequent sexual activity or masturbation.

3. Hormonal Changes: As men age, hormonal changes can affect sexual function and libido. While you are still relatively young, fluctuations in testosterone levels can influence sexual desire and performance.

4. Psychological Factors: Anxiety, stress, and performance pressure can contribute to sexual dysfunction. If you have been feeling anxious about your sexual performance or have been focusing on delaying ejaculation, this could lead to difficulties in achieving or maintaining an erection.

Symptoms and Their Implications
The symptoms you describe—such as discomfort in the bladder area after urination and difficulties with erection—are common in prostatitis. The fact that you do not experience pain during urination suggests that it may not be a straightforward case of urethritis. Instead, the discomfort could be related to the prostate itself or pelvic floor tension.

1. Erectile Dysfunction (ED): The issues with achieving and maintaining an erection, particularly after periods of increased sexual activity, could indeed indicate a form of sexual dysfunction. This can be multifactorial, involving both physical and psychological components. It is not uncommon for men to experience fluctuations in erectile function, especially if they are concerned about performance or have experienced discomfort in the past.

2. Impact of Sexual Activity: Engaging in frequent sexual activity or masturbation can lead to temporary changes in sexual function. If you have been trying to prolong sexual encounters or have been anxious about performance, this could contribute to the difficulties you are experiencing. The body can respond to stress and anxiety with physical symptoms, including changes in libido and erectile function.

Recommendations
1. Consult a Specialist: It may be beneficial to consult a urologist who specializes in prostatitis and sexual health. They can provide a thorough evaluation, including potential tests to rule out infections or other underlying conditions.

2. Pelvic Floor Therapy: Consider pelvic floor physical therapy, which can help address muscle tension and improve pelvic health. This approach can be particularly effective for chronic pelvic pain syndromes.

3. Lifestyle Modifications: Focus on maintaining a healthy lifestyle, including regular exercise, a balanced diet, and stress management techniques. Reducing alcohol and caffeine intake may also help alleviate symptoms.

4. Open Communication: If you are in a relationship, discussing your concerns with your partner can help alleviate performance anxiety and improve intimacy.

5. Psychological Support: If anxiety or stress is a significant factor, consider speaking with a mental health professional who can help address these concerns.

In summary, while prostatitis can be a challenging condition with various contributing factors, understanding its causes and implications for sexual health can empower you to seek appropriate treatment and support. Addressing both the physical and psychological aspects of your symptoms will be crucial in managing your condition effectively.
