Is it pregnancy?

Due to my girlfriend's menstrual cycle being very regular in the past, she was concerned when it was late this month.
Initially, we thought it might be due to stress from her recent job change.
After a week of delay, she purchased a pregnancy test, which showed negative results.
However, after another week, she took another test that indicated a positive result.
We then visited a gynecologist for an ultrasound, but nothing was visible.
The doctor did find a cyst approximately 10 centimeters in size on her ovary.
Initially, the doctor stated that she was not pregnant, but after mentioning the positive pregnancy test, the doctor suggested using a transvaginal ultrasound.
Initially, nothing was seen, but later a point was detected, which the doctor suggested could be the embryo, although it was noted that it had not yet implanted.

I would like to ask if pregnancy tests are reliable? Is she pregnant, or could this be something else? Given that her menstrual cycle is usually very regular, how can there be a two-week delay with the embryo not yet implanted?

Shan, 30~39 year old female. Ask Date: 2021/04/13

Hello, please take another pregnancy test.
If the result is negative, wait another week.
If your period still hasn't arrived, it is advisable to see a gynecologist.
Wishing you good health.

When it comes to understanding pregnancy test results and ovarian cysts, especially in the context of a missed period and fluctuating test results, it can be quite confusing. Let's break down the situation based on the details provided.

First, it's important to understand how pregnancy tests work. Most home pregnancy tests detect the hormone human chorionic gonadotropin (hCG), which is produced shortly after a fertilized egg attaches to the uterine lining. Typically, hCG can be detected in urine about 14 days after conception, which corresponds to the time of a missed period for many women. However, the sensitivity of the test can vary by brand, and testing too early can lead to false negatives.

In your case, your girlfriend's menstrual cycle is usually regular, and the delay in her period could indeed be a sign of pregnancy. The fact that the first test showed negative and the second test showed positive could suggest that she tested too early the first time. It's also possible that stress from changing jobs could have affected her cycle, leading to a delayed period.

The ultrasound findings of a 10 cm cyst in the ovary are significant. Ovarian cysts are fluid-filled sacs that can form on the ovaries and are quite common, especially in women with conditions like polycystic ovary syndrome (PCOS). While many cysts are benign and resolve on their own, larger cysts can sometimes cause complications or symptoms, such as pain or pressure.

The doctor’s initial assessment of no pregnancy, followed by the suggestion of a transvaginal ultrasound, indicates that they are taking a thorough approach to evaluate the situation. The fact that the doctor mentioned seeing a "point" that might indicate a pregnancy but noted that it hasn't implanted yet suggests that there may be a very early pregnancy. This is not uncommon; sometimes, implantation can take longer than expected, especially in cases of irregular cycles or hormonal imbalances.

Regarding the accuracy of pregnancy tests, they are generally reliable when used correctly. However, false negatives can occur, particularly if the test is taken too early. If your girlfriend is still experiencing symptoms of pregnancy (like nausea, breast tenderness, or fatigue) and her period does not start, it would be advisable to follow up with her healthcare provider for further testing, possibly including a blood test for hCG, which can provide a more definitive answer.

In summary, while the positive pregnancy test suggests that there may be a pregnancy, the presence of a large ovarian cyst and the doctor's comments about the implantation status indicate that further monitoring is essential. It's crucial to keep in close contact with her healthcare provider to ensure that both her reproductive health and any potential pregnancy are managed appropriately. If she continues to experience symptoms or if there are any concerns about the cyst, additional imaging or intervention may be necessary.
