Is a delayed menstrual period a sign of pregnancy?

Hello Doctor, my girlfriend's last menstrual period was from February 26 to March 2.
We had sexual intercourse on March 1 when her flow was light, and again on March 26 (using a condom that did not break).
My girlfriend is expected to have her next period on March 26 (due to polycystic ovary syndrome, her cycles are usually about a week late).
On April 5, a pregnancy test showed one line.
On April 8, my girlfriend mentioned that her breasts felt noticeably swollen and her nipples were painful, but this sensation only lasted for 2-3 days and then disappeared.
As of April 13, her period still hasn't arrived.
Doctor, what is the likelihood of her being pregnant?

Alex, 20~29 year old female. Ask Date: 2021/04/13

Hello, the chances of pregnancy should be low.
You can confirm with a pregnancy test.
Wishing you good health.

Late menstruation can indeed indicate pregnancy, but it is not the only reason for a missed period. In your girlfriend's case, several factors need to be considered to assess the likelihood of pregnancy.

Firstly, let's look at the timeline you've provided. Your girlfriend's last menstrual period (LMP) was from February 26 to March 2, with sexual intercourse occurring on March 1 and again on March 26. If her menstrual cycle is typically irregular due to polycystic ovary syndrome (PCOS), it is not uncommon for her period to be delayed. Women with PCOS often experience irregular cycles, which can make it challenging to predict ovulation and menstruation accurately.

The fact that she took a pregnancy test on April 5 and received a negative result (one line) is significant. Pregnancy tests work by detecting the hormone human chorionic gonadotropin (hCG) in urine, which is produced after a fertilized egg implants in the uterus. If the test is taken too early, it may not detect hCG levels yet, especially if ovulation occurred later than expected. However, if she had a negative test and is still experiencing a late period, it could suggest several possibilities:
1. Pregnancy: If ovulation occurred later than expected, it is possible that she could still be pregnant. If she has not had a period by now, it may be worth repeating the pregnancy test a few days later or consulting a healthcare provider for a blood test, which is more sensitive and can detect lower levels of hCG.

2. Hormonal Imbalance: Conditions like PCOS can lead to hormonal imbalances that may cause missed or delayed periods. This could explain the late menstruation without pregnancy.

3. Stress or Lifestyle Factors: Stress, significant weight changes, or changes in physical activity can also impact menstrual cycles. If your girlfriend has experienced any of these changes, they could contribute to her late period.

4. Other Medical Conditions: There are other medical conditions that can affect menstrual cycles, such as thyroid disorders or other hormonal issues. If her period does not arrive soon, it may be advisable for her to see a healthcare provider for further evaluation.

Regarding the breast tenderness and swelling she experienced, these can be early signs of pregnancy, but they can also occur due to hormonal fluctuations unrelated to pregnancy. The fact that these symptoms were brief (lasting only 2-3 days) may suggest they were not related to pregnancy.

In conclusion, while late menstruation can indicate pregnancy, especially if accompanied by other symptoms, it is not definitive. Given your girlfriend's history of irregular cycles and the negative pregnancy test, it is essential to consider other factors that may be at play. If her period does not arrive soon, or if she has any concerns, it would be wise for her to consult a healthcare provider for further assessment and possibly a more sensitive pregnancy test.
