Perineal mass
Hello, doctor.
As stated in the title, I have noticed a right-sided mass in the perineal area for several years, which I discovered while wiping after using the restroom.
It is slightly painful upon pressure and gives a sensation of a foreign body, but it has not ruptured, so there are no signs of drainage.
Recently, I have noticed that the mass has increased in size.

Dr. Xu Kaixi reply Surgery
Based on your description, this perineal mass may be a subcutaneous cyst or a lipoma.
It is recommended to visit a surgical outpatient clinic for further examination and confirmation.

More Info
Understanding a perineal mass can be crucial for determining the underlying cause and appropriate treatment options. A perineal mass refers to any abnormal growth or lump located in the perineal area, which is the region between the anus and the genitals. The causes of perineal masses can vary widely, and they may include benign conditions, infections, or even malignancies.
Causes of Perineal Masses
1. Cysts: One of the most common causes of a perineal mass is a cyst, such as a sebaceous cyst or a pilonidal cyst. These are typically benign and can occur due to blocked sebaceous glands or hair follicles.
2. Abscesses: An abscess is a collection of pus that can form due to infection. Although you mentioned that there is no discharge or signs of rupture, it is still possible for an abscess to form without overt symptoms initially.
3. Hemorrhoids: Swollen blood vessels in the rectal area can sometimes present as a mass. They can be painful and may cause discomfort during bowel movements.
4. Fistulas: An abnormal connection between the rectum and the skin can lead to the formation of a mass. This is often associated with chronic inflammation or infection.
5. Tumors: Both benign and malignant tumors can present as perineal masses. Benign tumors such as lipomas (fatty tumors) or fibromas can occur, but it is essential to rule out malignancies, especially if there is a change in size or symptoms.
6. Lymphadenopathy: Enlarged lymph nodes in the groin area can also present as a mass. This can occur due to infections, inflammatory conditions, or malignancies.
Symptoms
The symptoms associated with a perineal mass can vary depending on the underlying cause. In your case, you mentioned:
- Pain: Mild pain upon pressure can indicate inflammation or irritation in the area.
- Foreign Body Sensation: This can be a common symptom associated with cysts or other growths.
- Size Change: Noticing that the mass has increased in size is significant and warrants further evaluation.
Diagnosis
To accurately diagnose the cause of a perineal mass, a healthcare provider may perform the following:
- Physical Examination: A thorough examination of the area can help identify the characteristics of the mass.
- Imaging Studies: Ultrasound or MRI may be used to assess the mass's size, location, and characteristics.
- Biopsy: If there is suspicion of malignancy, a biopsy may be necessary to determine the nature of the mass.
Treatment Options
The treatment for a perineal mass will depend on the underlying cause:
1. Observation: If the mass is benign and asymptomatic, a "watch and wait" approach may be taken.
2. Medications: For infections or inflammatory conditions, antibiotics or anti-inflammatory medications may be prescribed.
3. Surgical Intervention: If the mass is causing significant discomfort, is growing, or is suspected to be malignant, surgical removal may be necessary.
4. Drainage: If an abscess is present, drainage may be required to relieve pain and prevent further complications.
Conclusion
In summary, a perineal mass can arise from various causes, including cysts, abscesses, hemorrhoids, tumors, and lymphadenopathy. Given that you have noticed an increase in size and experience mild pain, it is essential to consult a healthcare provider for a thorough evaluation. Early diagnosis and appropriate management are crucial to ensure the best outcomes and to rule out any serious conditions.
