Abnormal bleeding

Hello Doctor, my last menstrual period was from March 29 to April 1, which was nearly a week late compared to my usual cycle, and the flow was lighter.
However, I was quite busy with work in March, so I thought it was just due to fatigue and didn't pay much attention to it.
Since I experienced a lighter and delayed period last month, I took a packet of "Wenjing Decoction" yesterday, as it was around my ovulation period.
Unexpectedly, this morning I started bleeding similarly to my menstrual flow (with blood clots), and the amount is about the same as the one or two days before my period; I am still experiencing the flow.
I'm wondering if this could be ovulatory bleeding? What symptoms should I monitor to decide whether to seek medical attention? Recent sexual activities were on April 3, April 4, April 10, and April 11, all of which were safe.
Thank you.

Abnormal bleeding can be a concerning issue for many women, and understanding its causes and when to seek medical advice is crucial for maintaining reproductive health. Based on your description, it appears that you are experiencing a significant change in your menstrual cycle, which can be attributed to various factors, including hormonal fluctuations, stress, lifestyle changes, or even underlying medical conditions.

Firstly, let's clarify what constitutes abnormal bleeding. Abnormal bleeding refers to any bleeding that occurs outside of your normal menstrual cycle, including bleeding between periods, heavy menstrual bleeding, or bleeding after sexual intercourse. In your case, the bleeding you experienced after taking "温经汤" (a traditional Chinese herbal remedy) could potentially be related to hormonal changes or the effects of the herbal treatment on your body.

The timing of your bleeding, occurring around your ovulation period, raises the possibility of ovulation bleeding. Ovulation bleeding is typically light and may last for a few hours to a couple of days. It occurs due to the hormonal changes that happen when an egg is released from the ovary. However, the presence of blood clots and the volume of bleeding you described, which is similar to your menstrual flow, suggests that it may not be typical ovulation bleeding.

Several factors could contribute to your abnormal bleeding:
1. Hormonal Imbalance: Stress, changes in weight, and lifestyle factors can lead to hormonal fluctuations that may disrupt your menstrual cycle.
2. Infection or Inflammation: Conditions such as pelvic inflammatory disease (PID) or sexually transmitted infections (STIs) can cause abnormal bleeding.

3. Uterine Conditions: Polyps, fibroids, or endometriosis can lead to irregular bleeding patterns.

4. Medication Effects: If you are taking any medications, including herbal supplements, they may influence your menstrual cycle.

Given your recent history of stress and the changes in your menstrual cycle, it is essential to monitor your symptoms closely. Here are some guidelines on when to seek medical advice:
- Duration of Bleeding: If the bleeding lasts longer than a few days or becomes heavier than your usual menstrual flow, it is advisable to consult a healthcare provider.

- Associated Symptoms: If you experience severe pain, dizziness, or any other concerning symptoms, seek medical attention promptly.

- Frequency of Abnormal Bleeding: If this becomes a recurrent issue, it is essential to have a thorough evaluation to rule out any underlying conditions.

- Pregnancy Concerns: Since you mentioned recent sexual activity, if there is any possibility of pregnancy, a pregnancy test may be warranted, especially if your bleeding is unusual for your cycle.

In conclusion, while some instances of abnormal bleeding can be benign, it is crucial to pay attention to your body and seek medical advice when necessary. Keeping a detailed record of your menstrual cycle, including the timing, duration, and characteristics of any bleeding, can be helpful for your healthcare provider in diagnosing any potential issues. If your symptoms persist or worsen, do not hesitate to reach out to a gynecologist for further evaluation and management. Your health and well-being are paramount, and timely intervention can help address any concerns effectively.
