Nipple Issues: Causes, Concerns, and When to Seek Help - Surgery

Nipple issues

Recently, I experienced itching in my right breast, which improved significantly after applying a topical ointment prescribed by a dermatologist.
However, the size of my right nipple is still slightly larger than the left, and it appears uneven with several indentations resembling those made by fingernails, along with uneven coloration and mild flaking around the areola.
Aside from this, there are no symptoms such as lumps or discharge.
I would like to ask the doctor: Is this a side effect of the steroid ointment or a normal phenomenon? Is it normal for the sizes of the nipples to differ? Should I be concerned about issues such as male breast cancer or Paget's disease?

Niming, 10~19 year old female. Ask Date: 2021/04/17

Dr. Tang Yuxiong reply Surgery

Hello: Male breast cancer is generally less common.
Based on the description, it may be a localized skin issue.
It is recommended to follow up with a dermatologist for evaluation and treatment of the condition.
Abnormal nipple changes include unusual discharge, retraction, or palpable lumps.
If none of these symptoms are present, observation is advised.
However, if there are still concerns, it is recommended to visit the breast surgery department for further evaluation.

More Info

When it comes to nipple issues, it’s important to understand that variations in size, shape, and color can be quite common and often do not indicate a serious health problem. However, it’s also crucial to be aware of any changes that might warrant further investigation. Based on your description, let’s break down the concerns you’ve raised.

Causes of Nipple Changes
1. Size and Shape Variations: It is entirely normal for one nipple to be slightly larger or differently shaped than the other. Many people experience asymmetry in their bodies, including the breasts and nipples. This can be due to natural anatomical differences, hormonal changes, or even previous injuries.

2. Skin Irritation: The itching you experienced in your right breast that improved with the application of a topical corticosteroid (like betamethasone) suggests that you may have had a mild inflammatory skin condition, such as eczema or contact dermatitis. These conditions can cause symptoms like itching, redness, and scaling.

3. Skin Texture Changes: The presence of grooves or indentations on the nipple can be a result of various factors, including skin irritation or even normal anatomical variations. However, if these changes are new or have progressed, they should be monitored.

4. Color Changes: Variations in color, especially if they are accompanied by other symptoms, can sometimes indicate underlying issues. However, if the color change is subtle and there are no other alarming symptoms (like discharge or significant pain), it may not be a cause for concern.

When to Seek Help
While many nipple changes are benign, certain symptoms should prompt a visit to a healthcare provider:
- Persistent Changes: If the size difference becomes more pronounced or if the texture changes significantly over time.

- Discharge: Any unusual discharge from the nipple, especially if it is bloody or has an unusual color or odor.

- Pain or Discomfort: If you experience persistent pain or discomfort in the breast or nipple area.

- Skin Changes: If the skin around the nipple becomes red, swollen, or develops sores that do not heal.

- Family History: If you have a family history of breast cancer or other breast diseases, it’s wise to be more vigilant about changes.

Concerns About Male Breast Cancer and Paget's Disease
- Male Breast Cancer: While male breast cancer is rare, it can occur. Symptoms may include a lump in the breast tissue, changes in the nipple or breast skin, and discharge. Given that you do not have a lump or discharge, the likelihood is lower, but it’s still essential to monitor any changes.

- Paget's Disease of the Nipple: This is a rare form of breast cancer that typically presents with symptoms such as redness, scaling, and discharge from the nipple. If you notice any worsening of your symptoms or new symptoms developing, it would be prudent to consult a healthcare professional.

Conclusion
In summary, while the variations in your nipple size and the skin changes you describe may be normal, it’s essential to remain vigilant. Keep an eye on any changes, and don’t hesitate to reach out to a healthcare provider if you have concerns. Regular check-ups and being aware of your body’s changes are key components of maintaining breast health. If you feel uncertain or anxious about your symptoms, seeking a professional evaluation can provide peace of mind and ensure that any potential issues are addressed promptly.
