Balanoposthitis

Hello Doctor, I am obese, and my last sexual activity was two years ago.
I have phimosis, but I can retract it when erect.
I experienced inflammation for the first time.
On March 29, I visited a doctor due to itching around the foreskin, and I was prescribed Meikang ointment, which resolved the issue in a week.
On April 7, while masturbating, I noticed a red spot on the glans that was painful to touch.
I thought it was a wound, but it grew larger, and I went back to see the same doctor on April 12, who diagnosed me with balanitis and prescribed Xincimycin.
He advised me to apply it or alternate it with Meikang.
Initially, there was only one spot, but during the treatment, more spots appeared.
I noticed small raised dots that turned red and looked like ulcers, with about 5 or 6 wounds appearing in a spiral pattern.
As of today, only the initial area has shown improvement, while the others feel unchanged, forming a continuous scar on the glans.
The doctor mentioned that it is serious and may require electrocautery, which makes me hesitant to continue with him and consider changing doctors.
I had my fasting blood sugar tested, which was 84.

1.
If there is no improvement, should I return for a follow-up next week?
2.
I read in previous notes that the doctor said after applying the ointment, I should retract the foreskin, and it is not necessary to keep the glans exposed regularly, correct?
3.
After showering, I lie on the bed with the glans exposed, using a fan to dry the area before applying the ointment.
Is this acceptable? Thank you, Doctor.

Hello: 1.
If there is no improvement, should I return for a follow-up appointment next week? --> Take good care of your skin; there will be improvement, and a follow-up is necessary.
2.
I read the previous notes, and the doctor said to retract the foreskin after applying the medication, and that it is not necessary to keep the glans exposed regularly, right? (It can be covered, but it must be clean and dry before applying the medication.) 3.
After showering, if I lie on the bed and expose the glans while using a fan to dry the area, and then apply the medication, is that acceptable? --> Rinse with clean water, do not use soap, do not use a fan; gently pat dry.
If you have ointment, apply it twice a day; if it improves, once a day; if severe, three times a day.
If it feels damp after urination, use tissue to dry it; it is best to avoid moisture as it will slow healing.
Maintain hygiene during masturbation as well.
Pay attention to rest when there are lesions.
Wishing you good health.
P.S.
Maintaining cleanliness and dryness is best; especially in hot or cold weather, conditions can lead to skin infections.

More Info

Balanitis is an inflammation of the glans (the head) of the penis, which can be caused by various factors including infections, irritants, and underlying medical conditions. Understanding the causes, symptoms, and treatment options for balanitis is crucial for effective management and prevention of recurrence.

Causes of Balanitis
1. Infections: The most common cause of balanitis is infections, which can be fungal (like Candida), bacterial, or viral. Poor hygiene, especially in uncircumcised men, can lead to the accumulation of smegma, which can irritate the glans and promote infection.

2. Irritants: Chemical irritants from soaps, lotions, or condoms can cause allergic reactions or irritation leading to balanitis.
3. Skin Conditions: Conditions such as psoriasis, eczema, or lichen sclerosus can also affect the genital area and lead to inflammation.

4. Diabetes: Men with diabetes are at a higher risk of developing balanitis due to higher glucose levels in urine, which can promote yeast infections.

Symptoms of Balanitis
Symptoms of balanitis can vary but typically include:
- Redness and swelling of the glans
- Itching or burning sensation
- Pain or discomfort, especially during urination or sexual activity
- Discharge from the penis
- Ulcerations or sores on the glans

Treatment Options
Treatment for balanitis depends on the underlying cause:
1. Antifungal Creams: If a fungal infection is diagnosed, antifungal creams such as clotrimazole or miconazole may be prescribed.

2. Antibiotics: For bacterial infections, topical or oral antibiotics may be necessary. In your case, the doctor prescribed "欣黴素" (likely a form of clindamycin), which is effective against certain bacterial infections.

3. Steroid Creams: In cases of inflammation without infection, topical steroids can help reduce inflammation and irritation.

4. Good Hygiene Practices: Maintaining proper hygiene is crucial. For uncircumcised men, gently retracting the foreskin and cleaning the area regularly can help prevent the buildup of smegma and reduce the risk of infection.

5. Avoiding Irritants: Identifying and avoiding any irritants, such as certain soaps or lotions, can help prevent recurrence.

6. Surgical Options: In severe or recurrent cases, circumcision may be considered as a definitive treatment.

Addressing Your Concerns
1. Follow-Up Appointment: If you notice no improvement or if the condition worsens, it is advisable to return to your doctor for further evaluation. Persistent symptoms may require a different treatment approach or further investigation.

2. Hygiene Practices: After applying medication, it is generally recommended to cover the area to protect it from irritation. However, keeping the area clean and dry is essential. If your doctor advised that it is okay to keep the glans exposed after applying the medication, then you can follow that guidance.

3. Drying and Medication Application: It is acceptable to let the area dry after bathing before applying medication. Ensure that the area is clean and dry to promote healing and reduce the risk of further irritation.

Conclusion
Balanitis can be a distressing condition, but with appropriate treatment and hygiene practices, it can often be effectively managed. If you have ongoing concerns or if the condition does not improve, seeking a second opinion or further evaluation from a specialist may provide additional insights and treatment options. Always communicate openly with your healthcare provider about your symptoms and any changes you observe.
