Eczema issues
I have had eczema issues in my groin area since childhood.
I didn't pay much attention to it, but later I noticed that my skin became thickened.
I have seen a dermatologist twice; one prescribed steroids, while the other only prescribed an anti-itch ointment.
Recently, the originally itchy area stopped itching, but now the area under my penis is itchy.
Because my nails are long, I unconsciously scratched the skin while sleeping, and it has now developed scabs.
I would like to ask the doctor if my condition is related to heat and wearing jeans, as it only itches at night and during hot weather, but not in winter.
Could this be a fungal infection? If it is a fungal infection, will keeping the area dry help it heal on its own?

Dr. Du Shixi reply Urology
Hello: Skin infections can be difficult to heal, and immunity plays a role.
Dampness and heat can also create an environment conducive to recurrence.
Paying attention to these factors can lead to improvement.
If there is no redness or itching, it is possible to monitor the condition on your own.
However, there are situations that require medical attention.
Wishing you good health.

More Info
Eczema in the groin area, often referred to as "groin eczema" or "intertrigo," can be a frustrating and uncomfortable condition. It is characterized by inflammation of the skin, leading to symptoms such as itching, redness, and thickened skin. Understanding the causes, treatments, and care tips for this condition is essential for effective management.
Causes of Groin Eczema
1. Moisture and Heat: The groin area is prone to moisture and heat, especially in warm weather or when wearing tight clothing like jeans. This environment can lead to irritation and exacerbate eczema symptoms.
2. Friction: Tight clothing can cause friction against the skin, leading to irritation. This is particularly true for activities that involve a lot of movement.
3. Allergens and Irritants: Certain soaps, detergents, or personal care products can irritate the skin. Additionally, allergens such as fragrances or dyes can trigger eczema flare-ups.
4. Fungal Infections: The groin area is also susceptible to fungal infections, such as tinea cruris (jock itch), which can present with similar symptoms. This condition thrives in warm, moist environments and can cause intense itching and redness.
5. Skin Conditions: Other skin conditions, such as psoriasis or contact dermatitis, may also contribute to symptoms in the groin area.
Treatments for Groin Eczema
1. Topical Steroids: As you mentioned, your dermatologist prescribed a topical steroid. These medications help reduce inflammation and itching. It's important to use them as directed, as overuse can lead to skin thinning.
2. Antihistamines: Over-the-counter antihistamines can help relieve itching, especially if it disrupts your sleep.
3. Moisturizers: Regularly applying a fragrance-free moisturizer can help maintain skin hydration and barrier function, reducing the likelihood of flare-ups.
4. Antifungal Creams: If a fungal infection is suspected, antifungal creams may be necessary. It's crucial to consult a healthcare provider for an accurate diagnosis.
5. Avoiding Irritants: Identifying and avoiding potential irritants, such as certain fabrics or personal care products, can help manage symptoms.
Care Tips
1. Keep the Area Dry: After bathing or sweating, ensure the groin area is thoroughly dried. Consider using a powder designed for moisture control to help keep the area dry.
2. Wear Loose Clothing: Opt for loose-fitting, breathable fabrics to minimize friction and allow for air circulation.
3. Maintain Hygiene: Regular bathing with mild, fragrance-free soap can help keep the area clean without causing irritation.
4. Avoid Scratching: While it can be difficult, try to avoid scratching the affected area, as this can lead to further irritation and potential infection.
5. Monitor Symptoms: Keep track of any changes in your symptoms. If you notice increased redness, swelling, or discharge, it may indicate a secondary infection that requires medical attention.
Conclusion
Your symptoms of itching and thickened skin in the groin area could indeed be related to eczema, exacerbated by moisture and friction from clothing. If you suspect a fungal infection, maintaining dryness and possibly using antifungal treatments may be beneficial. However, it's essential to consult with a healthcare provider for an accurate diagnosis and tailored treatment plan. If your symptoms persist or worsen, further evaluation may be necessary to rule out other underlying conditions.
