Blood-stained toilet?
Hello, doctor.
Two to three years ago, I had an issue with rectal bleeding and underwent a colonoscopy, which showed no abnormalities.
The doctor at that time suggested that the bleeding might be related to hemorrhoids and advised me to schedule a separate appointment for further examination, so I did not pursue follow-up treatment.
Recently, I have been taking enzymes and frequently using enemas, which help me have a bowel movement every one to two days.
However, I often experience significant bleeding during these bowel movements, with the toilet bowl turning completely red.
After wiping, I still notice several drops of blood on the floor when I stand up, and this condition can last for several days.
Lately, in addition to the toilet being stained red, I have also observed large blood clots (similar to the thick, red discharge during menstruation) on the toilet paper.
Could this be a hemorrhoid issue, or do I need to undergo another colonoscopy?

When it comes to the presence of blood in stool, it is essential to approach the situation with caution and seek appropriate medical evaluation. Based on your description, it appears that you have a history of rectal bleeding, which was previously attributed to hemorrhoids after a colonoscopy showed no abnormalities. However, the recent escalation in the volume of blood, the presence of blood clots, and the consistency of the blood resembling menstrual discharge warrant further investigation.
Firstly, while hemorrhoids can indeed cause significant bleeding, especially during bowel movements, the amount of blood you are describing—where the toilet water turns completely red and blood continues to drip after wiping—suggests that there may be more than just hemorrhoids at play. Other potential causes of rectal bleeding include anal fissures, diverticular disease, inflammatory bowel disease (IBD), colorectal polyps, or even colorectal cancer, particularly given the concerning symptoms of blood clots and the duration of the bleeding.
Given that you have been experiencing these symptoms for several days and they have worsened, it is advisable to undergo another colonoscopy or at least a thorough evaluation by a gastroenterologist. This will help rule out any serious conditions and provide a clearer understanding of the underlying cause of your symptoms. It is important to note that while hemorrhoids are a common cause of rectal bleeding, they are not the only cause, and the presence of blood should always be taken seriously.
Additionally, the use of enemas and enzymes, while they may help with bowel movements, could potentially irritate the rectal area further, especially if there are existing hemorrhoids or fissures. It is crucial to discuss your bowel habits and any treatments you are using with your healthcare provider, as they may recommend alternative methods to manage your symptoms without exacerbating the bleeding.
In summary, while hemorrhoids could be a contributing factor to your symptoms, the severity and nature of the bleeding you are experiencing suggest that further investigation is necessary. A repeat colonoscopy or consultation with a gastroenterologist is recommended to ensure that any serious conditions are ruled out and to receive appropriate treatment for your symptoms. Please prioritize your health and seek medical attention promptly to address these concerning symptoms.
