Menstrual issues

Hello Doctor: My menstrual cycle has been irregular since I started menstruating, and it has been about ten years since then.
The irregularity has become increasingly severe.
I took ovulation medication for three months; the first attempt was ineffective, and after the doctor increased the dosage, I had two menstrual cycles, but the third attempt was also ineffective.
The doctor advised me to stop the medication and observe, and prescribed me a menstrual induction medication.
My period started on February 1st, but the flow was very light.
At the end of February, I experienced symptoms similar to premenstrual syndrome, but my period did not come.
By the end of April, I continued to have intermittent symptoms (fatigue, lower abdominal pain, breast tenderness), so I went to a gynecologist for an examination.
After an ultrasound, the doctor said that my endometrium was not thickened and there were no signs of ovulation, so I was prescribed a five-day course of menstrual induction medication.
I started taking it on April 18th until April 22nd, and now it has been nine days, but my period still has not arrived.
Around the fourth day of taking the medication, I began to experience premenstrual syndrome symptoms, but they are gradually subsiding now.
I would like to ask the doctor if this situation requires more detailed examinations.
Could there be issues like premature ovarian failure, or how can I improve this situation?

Emma, 20~29 year old female. Ask Date: 2021/05/01

Irregular menstrual cycles can be a source of concern for many women, especially when they experience symptoms such as fatigue, breast tenderness, and abdominal pain. Understanding the underlying causes, symptoms, and potential solutions is crucial for managing these irregularities effectively.

Firstly, it is important to recognize that menstrual cycles can vary significantly from woman to woman and can be influenced by a variety of factors, including hormonal imbalances, stress, lifestyle changes, and underlying medical conditions. In your case, the irregularity has persisted for about ten years, which suggests that there may be a chronic issue at play.

The symptoms you describe, such as fatigue, abdominal pain, and breast tenderness, are commonly associated with premenstrual syndrome (PMS) and can occur even when menstruation does not follow. This can be particularly frustrating, as it may feel like your body is preparing for a period that does not arrive. The fact that you have been prescribed ovulation-inducing medication and have had mixed results indicates that your healthcare provider is actively trying to address the hormonal aspects of your cycle.

The absence of a thickened endometrial lining and lack of ovulation signs on ultrasound suggests that your body may not be responding normally to hormonal signals. This could be indicative of conditions such as polycystic ovary syndrome (PCOS), hypothalamic amenorrhea, or even premature ovarian insufficiency (early menopause), especially if you are experiencing symptoms like fatigue and irregular cycles.

Given that you have already undergone some treatment and monitoring, it may be beneficial to pursue further evaluation. This could include blood tests to measure hormone levels (such as estrogen, progesterone, luteinizing hormone, and follicle-stimulating hormone) to assess ovarian function and rule out conditions like thyroid dysfunction or hyperprolactinemia, which can also affect menstrual regularity.

In terms of management, lifestyle modifications can play a significant role in regulating menstrual cycles. Regular exercise, a balanced diet, and stress management techniques such as yoga or meditation can help improve hormonal balance. Additionally, if you are experiencing significant stress or changes in your life, addressing these factors may also help restore regularity to your cycles.

If hormonal treatments have not been effective, your healthcare provider may consider alternative options, such as hormonal contraceptives, which can help regulate cycles and alleviate symptoms associated with PMS. These medications can provide a more predictable cycle and reduce the discomfort associated with irregular periods.

In conclusion, while irregular menstrual cycles can be concerning, understanding the potential causes and seeking appropriate medical evaluation and treatment can help manage the symptoms effectively. It is essential to maintain open communication with your healthcare provider, discuss your symptoms in detail, and explore further diagnostic options to determine the best course of action for your specific situation. Regular follow-ups and monitoring will be key in addressing any underlying issues and improving your overall menstrual health.
