Anal abscess

Hello, doctor.
About a month ago, I experienced diarrhea, and after it resolved, I noticed a small bump near my anus that resembled a pimple (about 1 cm in size).
It later ruptured and bled, but the amount was minimal, so I didn't think much of it.
Recently, the wound has not healed, so I went to see a doctor.
The doctor prescribed two medications and an ointment, and after three days of warm sitz baths, it started to heal.
However, pus began to accumulate again and then ruptured, draining out.
After this drainage, the swelling in the affected area has decreased significantly.

1.
Is this a normal part of the healing process?
2.
Is there a possibility of a fistula forming? (I'm quite worried about needing surgery.) Thank you.

Duncan, 20~29 year old female. Ask Date: 2021/05/02

Dr. Ke Fangxu reply Surgery

1.
The lump may be a thrombosed hemorrhoid or an anal abscess, which typically resolves on its own after it ruptures and drains completely.
Regular warm sitz baths are recommended.

2.
If it is an anal abscess that has ruptured, there is indeed a possibility of it developing into a fistula.
It is advisable to wait until the wound stabilizes before consulting a physician for an examination.

Understanding anal abscesses and their healing process can be quite concerning, especially when complications such as fistulas are a possibility. Based on your description, it seems like you have experienced an anal abscess, which is a localized infection that can occur in the anal region. Let’s break down your concerns and provide some clarity on the healing process and the potential for fistula formation.

1. Healing Process of an Anal Abscess:
The initial symptoms you experienced, including the small lump near the anus that eventually ruptured and bled, are typical of an anal abscess. The fact that you noticed some improvement after treatment, including the use of medications and warm sitz baths, is a positive sign. It is common for abscesses to drain pus, and this drainage is part of the healing process. The accumulation of pus and subsequent drainage can lead to a reduction in swelling and pain. However, it’s also normal for the area to remain tender and for some swelling to persist during the healing process.
After an abscess drains, the tissue may take time to heal completely, and it’s not unusual for there to be some residual swelling or discomfort. The key is to monitor the area for any signs of worsening infection, such as increased redness, swelling, or fever, which would necessitate further medical evaluation.

2. Concerns About Fistula Formation:
One of the potential complications of an anal abscess is the development of a fistula. A fistula is an abnormal connection between the anal canal and the skin surrounding the anus, which can occur if the abscess does not heal properly. The risk of developing a fistula increases if the abscess is deep or if it recurs.
Given that you have experienced drainage and some improvement, it is essential to follow up with your healthcare provider to assess the healing process. They may perform a physical examination to determine if there is any evidence of a fistula. If a fistula is suspected, further evaluation, such as imaging studies or a referral to a specialist, may be necessary.

3. What to Expect Moving Forward:
Continue with the prescribed medications and warm sitz baths, as these can help promote healing and comfort. Keep the area clean and dry, and avoid any irritants. If you notice any new symptoms, such as increased pain, persistent drainage, or changes in the appearance of the wound, contact your healthcare provider promptly.

4. Surgical Considerations:
If a fistula does develop, surgical intervention may be required to properly treat it. This typically involves a procedure to lay open the fistula tract, allowing it to heal from the inside out. While surgery can sound daunting, it is often necessary to prevent recurrent infections and complications.

In summary, while the healing process of an anal abscess can involve some discomfort and drainage, it is crucial to monitor the situation closely. The potential for fistula formation is a valid concern, and maintaining open communication with your healthcare provider will ensure that you receive appropriate care and intervention if needed. Remember, early detection and treatment of complications can significantly improve outcomes.
