Foreskin Issues: When to Seek Urology Care for Discharge - Urology

There seems to be a problem with the foreskin, as there appears to be a tear and purulent discharge?

Hello doctor, about two or three days ago, I think I may have caused some irritation during a DIY activity, resulting in a small tear in the gap between the glans and the foreskin.
Now, every so often (not at fixed intervals), there is a purulent discharge in the space between the foreskin and the glans.
I'm concerned and would like to know if I need to urgently see a urologist or if it can heal on its own.

Niming, 20~29 year old female. Ask Date: 2021/05/03

Dr. Du Shixi reply Urology

Hello: 1) You should seek medical attention.
2) Currently, keep the area clean and dry, and avoid using soap or body wash to prevent irritation.
Wishing you good health.

When it comes to issues related to the foreskin and discharge, it is important to understand the underlying causes and when to seek medical attention. In your case, the presence of purulent discharge between the glans (the head of the penis) and the foreskin, especially following an injury, could indicate an infection or inflammation, commonly known as balanitis or posthitis.

Balanitis is an inflammation of the glans, while posthitis refers to inflammation of the foreskin. Both conditions can occur simultaneously, leading to discomfort, redness, swelling, and discharge. The purulent discharge you are experiencing is a sign that there may be an infection present, which could be bacterial or fungal in nature.

Here are some key points to consider regarding your situation:
1. Symptoms and Severity: If the discharge is accompanied by other symptoms such as significant pain, swelling, fever, or if the area appears increasingly red or inflamed, it is advisable to seek urological care promptly. These could be signs of a more serious infection that may require medical intervention.

2. Self-Care: If the symptoms are mild and you are not experiencing significant pain or discomfort, you might consider practicing good hygiene. Gently cleaning the area with warm water and avoiding irritants (such as soaps or lotions that may cause further irritation) can help. However, if the discharge persists or worsens, medical attention is warranted.

3. Potential Complications: Untreated infections can lead to complications such as phimosis (inability to retract the foreskin), recurrent infections, or even more serious conditions if the infection spreads. Therefore, monitoring your symptoms closely is crucial.

4. When to Seek Help: It is generally a good idea to consult a urologist if you notice any of the following:
- Persistent or worsening discharge
- Increased pain or discomfort
- Signs of systemic infection (fever, chills)
- Difficulty retracting the foreskin or painful urination
5. Diagnosis and Treatment: A urologist will likely perform a physical examination and may take a sample of the discharge for laboratory analysis to determine the cause of the infection. Treatment may involve topical or oral antibiotics, depending on the severity and type of infection.

6. Preventive Measures: Once treated, maintaining good hygiene and avoiding irritants can help prevent future occurrences. If you have recurrent issues, discussing potential underlying causes with your doctor is essential.

In conclusion, while minor cases may resolve with self-care, the presence of purulent discharge, especially following an injury, should not be ignored. If you have any doubts or if your symptoms change, it is always best to err on the side of caution and seek professional medical advice. Urologists specialize in these types of conditions and can provide the appropriate care and treatment to ensure your health and well-being.
