White Bumps on the Penis: Causes and Concerns - Urology

There are some white spots located along the underside of the penis, beneath the frenulum.
They have been present for a long time without increasing or decreasing in number.
I have no sexual experience and have only handled this myself.
I would like to know what these symptoms might indicate.
(It seems that my foreskin is slightly elongated, as I have to push it back during an erection to see the coronal sulcus.)

Dr. Chen Jiansheng reply Urology

It may be a cholesteatoma; observation is sufficient.

Understanding white bumps on the penis can be concerning, especially when they appear to be persistent and unchanging. Based on your description, it seems that you have noticed white spots located along the frenulum (the band of tissue on the underside of the penis) that have not changed in size or appearance over time. Given that you have no sexual experience and have only engaged in self-exploration, it is important to consider several potential causes for these white bumps.

One common condition that could explain the presence of white bumps is Pearly Penile Papules (PPP). These are small, benign growths that typically appear around the corona of the glans (the head of the penis) and are often mistaken for sexually transmitted infections or other medical conditions. PPP is a normal anatomical variant and is not associated with any health risks or sexually transmitted diseases. They are usually painless and do not require treatment unless they cause psychological distress.

Another possibility is Fordyce spots, which are small, painless, pale bumps that can appear on the shaft of the penis or the scrotum. These spots are sebaceous glands that are not associated with hair follicles and are completely normal. Like PPP, Fordyce spots are harmless and do not require any medical intervention.

If the white bumps you are experiencing are not consistent with PPP or Fordyce spots, other conditions could be considered. For instance, Pearly Penile Papules can sometimes be confused with Lichen Planus, a condition that can cause white patches or bumps on the skin and mucous membranes. Lichen Planus can be associated with discomfort and may require treatment if it causes symptoms.

Additionally, Balanitis or inflammation of the glans can lead to changes in the skin, including white patches or bumps. This condition can be caused by irritants, infections, or poor hygiene, and it may require treatment to alleviate symptoms.

Given that you mentioned your foreskin appears to be slightly long, this could also contribute to hygiene issues, which might lead to irritation or infections. Proper hygiene is crucial, especially for individuals with a longer foreskin, as it can trap moisture and bacteria.

In conclusion, while the white bumps you are experiencing may be benign conditions such as Pearly Penile Papules or Fordyce spots, it is essential to consult a healthcare professional for an accurate diagnosis. A urologist or dermatologist can perform a physical examination and, if necessary, conduct further tests to rule out any other conditions. It is always better to seek medical advice when you notice changes in your body, especially in sensitive areas, to ensure peace of mind and proper health management.
