Questions regarding the penis and semen?
Hello Doctor, I would like to ask a few questions regarding the penis and semen.
1.
Recently, I have been experiencing an itchy sensation on the glans, and I feel like some fluid is leaking out.
However, when I check my underwear in the restroom, it is dry.
Is this normal?
2.
Sometimes, when I think about sexually stimulating things, my heart rate increases and I become erect.
In the past, I would only experience an erection, but recently I have noticed that sometimes, before I am fully erect (when the penis is only slightly hard and mostly soft), I feel a liquid flowing from the urethra.
The amount is not much.
Could this fluid be semen? When I checked my underwear today, I did not find any jelly-like fluid, only traces that looked like urine.
Is this phenomenon normal? Also, could this fluid cause pregnancy? If this fluid can lead to pregnancy, it would be very concerning for me, as I commute and need to maintain distance from others.
Thank you, Doctor.

Hello: The symptoms do not seem to indicate an allergy or a cold.
Typically, a sore throat from a cold does not resolve within an hour.
If the sore throat only occurs upon waking, it may be related to snoring or breathing through the mouth during sleep, or it could be due to dry air caused by air conditioning.
You can place a cup of water in the air-conditioned room or drink water after waking up.
If discomfort persists, it is advisable to seek medical evaluation.
Wishing you safety and health.

Understanding penile discomfort and semen leakage can be a source of concern for many individuals. Based on your questions, let's address each of your concerns in detail.
1. Itchy Sensation and Fluid Leakage: The itchy feeling you are experiencing on the glans (the head of the penis) could be due to several reasons, including irritation, an allergic reaction, or an infection. Common causes of itching in this area include fungal infections (like candidiasis), bacterial infections, or sexually transmitted infections (STIs) such as herpes or gonorrhea. The fact that you notice a fluid leakage but find your underwear dry could indicate that the fluid is not significant enough to soak through your clothing. This fluid could be pre-ejaculatory fluid, which is produced by the Cowper's glands and can sometimes be released during arousal or stimulation. If the itching persists or is accompanied by other symptoms such as redness, swelling, or pain, it is advisable to consult a healthcare professional for a proper diagnosis and treatment.
2. Fluid Release During Arousal: The liquid you describe as flowing out of your urethra when you are partially erect is likely pre-ejaculatory fluid, also known as pre-cum. This fluid is produced by the Cowper's glands and serves to lubricate the urethra and neutralize acidity in the urethra before ejaculation. It is important to note that while pre-ejaculatory fluid can contain sperm, the concentration is generally much lower than in ejaculate. However, it is still possible for pre-ejaculatory fluid to carry sperm, which means there is a potential risk of pregnancy if it comes into contact with a partner's vagina. If you are concerned about the possibility of pregnancy, it is crucial to practice safe sex by using condoms or other forms of contraception.
3. Normalcy of These Phenomena: The experiences you describe can be normal variations of sexual arousal and physiological responses. Many men experience pre-ejaculatory fluid release during arousal, and it is not uncommon for this to happen even if the erection is not fully firm. However, if you find that these occurrences are causing you distress or anxiety, especially in social situations, it may be helpful to discuss these feelings with a healthcare provider or a mental health professional. They can provide guidance on managing anxiety related to sexual health.
4. Seeking Medical Advice: If the itching, discomfort, or fluid leakage continues or worsens, it is essential to seek medical advice. A healthcare provider can perform a physical examination, take a detailed history, and possibly conduct tests to rule out infections or other underlying conditions. Early intervention can help prevent complications and provide peace of mind.
In summary, while the symptoms you describe can be common and often benign, they can also indicate underlying issues that may require attention. It is always best to err on the side of caution and consult a healthcare professional if you have concerns about your sexual health. They can provide personalized advice based on your specific situation and help you navigate any anxiety or discomfort you may be experiencing.
