Premature ejaculation treatment
Hello doctor, I believe I have a problem with premature ejaculation.
A few days ago, I visited a doctor who conducted blood tests and reported that my hormone levels and other data were normal.
However, the doctor only advised me to maintain good lifestyle habits (going to bed early, exercising regularly) and to return for a follow-up to monitor my condition, without prescribing any medication or treatment, only mentioning behavioral therapy.
I informed the doctor that I already have these habits: I go to bed at 10 PM and wake up at 6:30 AM, and I exercise 3-5 times a week for 40-60 minutes, yet I still experience premature ejaculation.
I have only engaged in sexual activity once, but when I try to train for longer duration during masturbation, I can't last more than a minute before ejaculating.
The doctor said that since I have not had recent sexual activity, they cannot prescribe medication and that behavioral therapy is the only option.
However, I hope to perform better during future sexual encounters, which is why I sought medical advice.
I would like to know if this approach to my condition is appropriate, and whether I should seek a second opinion elsewhere.
Thank you.

Theoretically, there is no issue, as the definition of premature ejaculation refers to the duration of vaginal intercourse rather than self-stimulation.

Premature ejaculation (PE) is a common condition that affects many men at some point in their lives. It is characterized by the inability to control ejaculation, leading to a shorter duration of sexual intercourse than desired. The experience can be frustrating and can impact self-esteem and relationships. Understanding the treatment options available is crucial for those seeking to manage this condition effectively.
From your description, it seems that you have already consulted a physician who has ruled out hormonal imbalances and suggested maintaining a healthy lifestyle, along with behavioral therapy. While these recommendations are valid, they may not address your immediate concerns about performance during sexual activity.
Treatment Options for Premature Ejaculation:
1. Behavioral Techniques: These include methods such as the "stop-start" technique or the "squeeze" technique. The stop-start method involves pausing sexual stimulation before ejaculation, allowing the excitement to subside before resuming. The squeeze technique involves squeezing the penis just before ejaculation to delay it. These methods can be practiced during masturbation to help improve control over time.
2. Topical Anesthetics: Over-the-counter creams or sprays containing numbing agents like lidocaine can be applied to the penis to reduce sensitivity and help delay ejaculation. It's important to follow the instructions carefully to avoid any adverse effects.
3. Medications: While your doctor may have been hesitant to prescribe medication due to your current lack of sexual activity, there are several options available for PE. Selective serotonin reuptake inhibitors (SSRIs), such as paroxetine or sertraline, are often prescribed off-label for PE because they can delay ejaculation as a side effect. Another option is the use of certain antidepressants that have been shown to help with PE.
4. Counseling and Therapy: Sometimes, psychological factors such as anxiety or stress can contribute to PE. Speaking with a therapist or counselor can help address these issues and improve sexual performance.
5. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises (similar to Kegel exercises) can improve control over ejaculation. These exercises can be beneficial for both men and women and can enhance sexual function.
6. Consulting a Specialist: If you feel that your current treatment plan is not addressing your concerns adequately, seeking a second opinion from a urologist or a sexual health specialist may be beneficial. They can provide a more tailored approach to your situation and discuss additional treatment options.
Conclusion: It is essential to communicate openly with your healthcare provider about your concerns and treatment preferences. If you feel that your needs are not being met, it is entirely reasonable to seek a second opinion or consult a specialist. Remember that PE is a common issue, and there are effective treatments available. With the right approach, many men find significant improvement in their condition, leading to a more satisfying sexual experience.
