Post-Hemorrhoid Surgery: Itchy Rectum and Discharge Concerns - Surgery

One year after hemorrhoid surgery, there is itching in the rectum along with discharge?

Hello, I had hemorrhoidectomy surgery at the end of June last year.
It wasn't until January to February of this year that I stopped experiencing bleeding during bowel movements.
However, I have been feeling intermittent itching in the area where the hemorrhoids were located, and there is a discharge that is clear to bright yellow in color, sometimes containing small fecal particles and having a fecal odor.
Occasionally, when my stool is a bit hard, the discharge has a slight pink tint.
When I insert my finger to apply ointment (strong hydrocortisone ointment), I can feel a somewhat hard area where the hemorrhoids used to be.
After applying the ointment, the itching and discharge significantly decrease or even stop.
I would like to ask if this situation is indicative of rectal dermatitis, an anal fistula, or another underlying condition? There are no noticeable lumps or external openings around the anus.
I originally planned to return to the surgical hospital for an examination, but I had to cancel due to the pandemic, so I am seeking help here.
Thank you.

The Strongly Applied Beauty Ointment contains salicylic acid and should not be used on the anal or rectal area.
It is recommended that you stop using it and observe whether your symptoms improve.
Anal itching may be caused by excessive wiping.
There are three important points to pay special attention to: 1.
Do not scratch to avoid causing an infection in the wounds.
2.
Temporarily avoid consuming spicy foods to prevent capsaicin from irritating the skin.
3.
Avoid using soap or body wash to clean the anal area to prevent excessive removal of the skin's natural oils.
If you can follow these guidelines, improvement can generally be seen in about a week.
If there is no improvement, you should consult a physician for examination and medication.
As for whether it is an anal fistula, further examination by a physician is necessary to confirm.

Post-hemorrhoid surgery symptoms such as itching, discharge, and occasional bleeding can be concerning for patients. Based on your description, it seems you are experiencing a combination of symptoms that could be attributed to several factors, including irritation of the rectal mucosa, possible infection, or even the formation of a fistula.

Firstly, it is not uncommon for patients to experience itching and discharge after hemorrhoid surgery. The surgical site can be sensitive and may take time to heal completely. The presence of a clear to yellow discharge, especially if it has a fecal odor, could indicate a few possibilities. One potential cause is the irritation of the rectal mucosa, which can occur due to residual inflammation from the surgery or from the passage of stool. If the stool is hard, it can exacerbate irritation and lead to further discomfort.

The pinkish discharge you mentioned, especially when associated with hard stools, could suggest minor bleeding from the surgical site or from the surrounding tissue. It is essential to ensure that your bowel movements are soft to prevent straining, which can lead to further irritation or injury to the healing tissue. Adequate hydration, a high-fiber diet, and possibly the use of stool softeners can help manage this issue.

The hard area you feel in the location of the previous hemorrhoid could be scar tissue or a thrombosed vein, which is not uncommon after such surgeries. However, if the area is hard and you are experiencing persistent symptoms, it is crucial to have it evaluated by a healthcare professional.
Regarding the possibility of a fistula, this is a condition where an abnormal connection forms between the rectum and the skin or another organ. Symptoms of a fistula can include persistent discharge, itching, and sometimes pain. The absence of external openings or unusual protrusions around the anus does not rule out the presence of a fistula, as they can sometimes be internal.

Given the complexity of your symptoms and the potential for complications, it is advisable to seek medical attention. While you mentioned that you had to cancel your follow-up appointment due to the pandemic, it is important to prioritize your health. If your symptoms worsen, or if you experience fever, increased pain, or significant changes in your discharge, you should seek immediate medical care.

In summary, while some degree of itching and discharge can be normal after hemorrhoid surgery, the specific characteristics of your symptoms warrant further evaluation. It is essential to maintain soft stools and good hygiene while monitoring your symptoms closely. If possible, reschedule your follow-up appointment with your surgeon or a gastroenterologist to discuss your concerns and receive appropriate care. Your health and comfort are paramount, and timely intervention can prevent further complications.
