Possibility of Pregnancy

Hello, doctor.
I would like to ask: My last menstrual period was from April 24 to April 28 (the previous one was from March 29 to April 2), with an average cycle length of about 27.28 days.
I had sexual intercourse twice on May 1, both times using condoms, and a post-coital water test showed no damage.
However, my menstrual period was due on May 25 and has not arrived, which is about 4 days late.
Normally, I experience cramping before my period, but this time the discharge has been minimal, just a little.
I took a pregnancy test with my first morning urine on May 25, which showed negative, with no faint T line.
Currently, I have not experienced the usual premenstrual symptoms, although I do have chest pain without breast tenderness.
I am still concerned about the possibility of pregnancy and the accuracy of the pregnancy test taken 24 days after intercourse.
Thank you.

Chiu, 20~29 year old female. Ask Date: 2021/05/25

Dr. Cao Guotao reply Obstetrics and Gynecology

When it comes to understanding pregnancy possibilities, timing and method of testing are crucial. Based on your menstrual cycle, which averages around 27 to 28 days, your last period was from April 24 to April 28. Given that you had unprotected sexual intercourse on May 1, you are within the window where conception could potentially occur, especially if you ovulated around mid-cycle.

The key to determining whether you might be pregnant lies in understanding the timing of ovulation and the sensitivity of pregnancy tests. Typically, ovulation occurs about 14 days before your next expected period. If we assume a 28-day cycle, ovulation would likely have occurred around May 10. Sperm can live inside the female reproductive tract for up to five days, which means that intercourse on May 1 could potentially lead to pregnancy if ovulation occurred shortly thereafter.

Now, regarding your missed period on May 25, it is important to note that menstrual cycles can sometimes be irregular due to various factors such as stress, hormonal changes, or health issues. A delay of a few days is not uncommon, and it does not automatically indicate pregnancy.

You mentioned that you took a home pregnancy test on the morning of May 25, which returned a negative result. Home pregnancy tests are generally reliable, especially when taken after a missed period. However, the accuracy of these tests can vary based on several factors, including the timing of the test and the sensitivity of the test itself. Most tests are designed to detect the hormone hCG (human chorionic gonadotropin), which is produced shortly after a fertilized egg attaches to the uterine lining. Testing too early, before hCG levels are sufficiently elevated, can lead to a false negative result.

Given that you tested negative and are experiencing some breast tenderness without the usual premenstrual symptoms, it is possible that your body is simply experiencing a hormonal fluctuation. If your period does not arrive within a week or so, it would be advisable to retest or consult with a healthcare provider for further evaluation.

In summary, while there is a possibility of pregnancy given the timing of your sexual activity, the negative pregnancy test suggests that you may not be pregnant. However, if your period does not arrive soon, consider retesting or seeking medical advice for peace of mind and to rule out any underlying issues. Remember, stress and anxiety can also impact your menstrual cycle, so try to stay relaxed during this waiting period.
