Urination issues, experiencing difficulty or straining during urination?

Hello doctor, I am currently 23 years old.
Previously, after ejaculation, I would always feel the urge to urinate, and I would have normal urination.
Recently, after ejaculation, I have not felt the urge to urinate.
Six hours later, I felt the need to go to the bathroom, but when I got there, I felt like I couldn't urinate.
The urge felt like it was building up, but I couldn't release it.
After waiting for about two minutes and applying some physical effort, I was able to slowly urinate a little.
I noticed something unusual, so I quickly drank 200cc of water and waited for three hours, but the same situation occurred again.
It takes about 60 seconds to urinate, and it feels obstructed.

Dr. Jian Weihong reply Urology

Hello: Insufficient water intake may be a cause.
It is recommended to consume at least 2500cc of fluids per day.
If you experience pain during urination or discomfort in the lower abdomen while urinating, you should seek medical evaluation, as there may be other underlying issues.
Wishing you good health.

Understanding urinary issues, especially after ejaculation, can be complex and multifaceted. Your experience of feeling the urge to urinate immediately after ejaculation, followed by a delayed urge and difficulty urinating, raises several important considerations.
Firstly, it is not uncommon for men to experience a sensation of needing to urinate after ejaculation. This can be attributed to the physiological changes that occur during sexual arousal and ejaculation. The pelvic floor muscles, which play a crucial role in both sexual function and urination, are engaged during ejaculation. After ejaculation, these muscles may still be in a state of contraction, leading to a temporary sensation of urgency.

However, your recent experience of not feeling the urge to urinate immediately after ejaculation, followed by a delayed urge and difficulty in urination, is concerning. This change in your urinary pattern could be indicative of several underlying issues. One possibility is that there may be some degree of bladder dysfunction or pelvic floor muscle dysfunction. The bladder may not be contracting effectively, which can lead to a sensation of urgency without the ability to void completely.

Another consideration is the possibility of a urinary tract infection (UTI) or inflammation of the urinary tract, which can cause changes in urination patterns, including urgency, frequency, and difficulty initiating urination. Additionally, conditions such as prostatitis (inflammation of the prostate gland) can also lead to similar symptoms, particularly in younger men.

Given that you have experienced this issue multiple times, it is advisable to seek a thorough evaluation from a healthcare professional, preferably a urologist. They may recommend several diagnostic tests, including a urinalysis to check for infection, a post-void residual volume test to assess how much urine remains in the bladder after urination, and possibly imaging studies to evaluate the urinary tract and bladder function.

In the meantime, there are some strategies you can employ to help manage your symptoms. Staying well-hydrated is important, as it can help dilute urine and reduce irritation. Additionally, practicing pelvic floor exercises (Kegel exercises) may help strengthen the muscles involved in urination and improve bladder control.

It is also essential to be mindful of any other symptoms you may be experiencing, such as pain during urination, blood in the urine, or persistent discomfort in the pelvic area. These symptoms warrant immediate medical attention, as they could indicate a more serious underlying condition.

In summary, while it is not uncommon to experience changes in urination patterns after ejaculation, the symptoms you are describing suggest that further evaluation is necessary. A urologist can provide a comprehensive assessment and guide you toward appropriate treatment options based on the underlying cause of your symptoms. Taking proactive steps to address your urinary issues can help improve your quality of life and alleviate any concerns you may have.
