The foreskin has become rough and discolored?
A few nights ago, I started experiencing itching in my scrotum (a dry itch).
After applying betamethasone ointment for the second and third days, the skin on my foreskin and glans (which comes into contact with the side of the scrotum) began to become rough and wrinkled, with a darker color.
The itching in the scrotum improved after the fifth or sixth day of applying the medication, but the peeling has not improved.
There are no visible lesions or pinpoint red spots (which I had over a year ago), and I have not had a circumcision, but my foreskin usually retracts to the coronal sulcus.
What could be the cause? Should I see a urologist? Thank you.

Hello: In your case, itching of the skin is often accompanied by flaking and peeling.
If there is no redness, do not use ointments.
Keep the scrotum clean and dry, and avoid using water that is too hot for washing.
Make sure to dry the area thoroughly.
Wear loose-fitting underwear and avoid spicy and irritating foods.
Give this a try, and if you notice any redness, seek medical attention.
Wishing you good health.

Rough and darkened foreskin can be concerning, especially when accompanied by itching and changes in skin texture. Based on your description, it seems you are experiencing symptoms that could be attributed to several potential causes, including skin irritation, fungal infections, or other dermatological conditions.
1. Skin Irritation: The use of topical medications like Betamethasone (a corticosteroid) can sometimes lead to skin thinning or irritation if used excessively or for prolonged periods. This could explain the rough texture and color changes in the foreskin. If the irritation is due to a reaction to the medication or an underlying condition, it may require a different approach to treatment.
2. Fungal Infections: Conditions like candidiasis (yeast infection) can cause itching, redness, and changes in skin texture. This is particularly common in warm, moist areas of the body, including the genital region. If the itching improves but the peeling persists, it might indicate a fungal infection that needs specific antifungal treatment.
3. Dermatitis: Contact dermatitis from soaps, detergents, or other irritants could also lead to similar symptoms. If you have recently changed any personal care products, this could be a factor.
4. Balanitis: This is an inflammation of the glans (the head of the penis) and can occur due to poor hygiene, infections, or skin conditions. Symptoms include redness, swelling, and sometimes discharge.
5. Psoriasis or Eczema: These chronic skin conditions can also affect the genital area, leading to rough, darkened skin and itching. They often require specific dermatological treatments.
Given that your symptoms have persisted for several days and have not fully resolved with over-the-counter treatment, it is advisable to see a urologist or a dermatologist. A healthcare professional can perform a thorough examination, possibly including a skin scraping or culture, to determine the exact cause of your symptoms. They may also provide a more targeted treatment plan based on the diagnosis.
When to See a Urologist:
- Persistent Symptoms: If symptoms like itching, roughness, or discoloration continue despite treatment.
- Discharge or Odor: If you notice any unusual discharge or a foul smell, this could indicate an infection that needs medical attention.
- Pain or Discomfort: If you experience pain during urination or sexual activity, this warrants a visit to a specialist.
- Changes in Sexual Function: Any changes in your ability to achieve or maintain an erection should also be discussed with a urologist.
Conclusion:
While some skin changes can be benign and self-limiting, persistent or worsening symptoms should not be ignored. A urologist can provide a comprehensive evaluation and appropriate management to ensure your health and comfort. Remember, early intervention can often lead to better outcomes, so seeking professional advice is a prudent step.
