Ear pain?
Hello, Doctor.
I work in a labor-intensive job and often inhale dust, which leads to irritation and symptoms such as a runny nose, a scratchy throat, and dry heaving.
I previously consulted a doctor who recommended using a nasal irrigation device, and my condition improved significantly.
However, after rinsing my nose, I often experience a sensation of pressure in my left ear and a sharp pain lasting about five minutes.
I would like to ask if this could be related to my past experience with otitis media during my military service when I had a cold.
Additionally, I experienced unexplained dizziness two years ago and saw several specialists until a neurologist diagnosed me with inner ear nerve inflammation.
Should I see a doctor again?

Dr. Ji Zhaoyun reply Otolaryngology
Hello, dizziness can have numerous potential causes: 1) If accompanied by a dull pain at the back of the head, it may suggest poor blood circulation in the head and neck.
2) If there are symptoms of ear fullness, ear pressure, or hearing loss, it may indicate sudden sensorineural hearing loss, Meniere's disease, or acute labyrinthitis.
3) If turning the head exacerbates the symptoms, it may suggest benign paroxysmal positional vertigo (BPPV).
Other possible diagnoses include vestibular neuritis, migraine-associated vertigo, orthostatic hypotension, hyperlipidemia, and brain lesions, among others.
It is advisable to seek further evaluation and treatment from an otolaryngologist or neurologist if symptoms persist.
Wishing you safety and good health.

Ear pain can be a complex issue, often stemming from various underlying conditions. Given your history of working in a dusty environment and experiencing symptoms like nasal congestion, throat irritation, and a previous diagnosis of middle ear inflammation (otitis media), it's essential to consider several factors that could contribute to your current ear discomfort.
Firstly, your symptoms of pressure and sharp pain in the left ear after nasal irrigation could indicate a few possibilities. One common cause is Eustachian tube dysfunction. The Eustachian tube connects the middle ear to the back of the throat and helps equalize pressure. When you experience nasal congestion or sinus issues, this tube can become blocked, leading to a feeling of pressure and pain in the ear. The sharp pain you describe may be due to sudden changes in pressure, especially after nasal irrigation, which can create a vacuum effect in the ear.
Additionally, your history of middle ear infections could play a role. Recurrent infections can lead to scarring or changes in the Eustachian tube's function, making it more susceptible to dysfunction. If you had a previous episode of inner ear nerve inflammation, this could also contribute to your current symptoms, as the inner ear is closely linked to balance and hearing.
Given your symptoms and medical history, it is advisable to seek further medical evaluation. While nasal irrigation has helped alleviate some of your symptoms, persistent ear pain and pressure warrant a thorough examination by an Ear, Nose, and Throat (ENT) specialist. They can perform a detailed examination, including otoscopy to check for any abnormalities in the ear canal and tympanic membrane (eardrum), and possibly audiometry to assess your hearing.
In addition to Eustachian tube dysfunction, other potential causes of ear pain include:
1. Sinusitis: Inflammation of the sinuses can lead to referred pain in the ears due to shared nerve pathways.
2. Allergic Rhinitis: Allergies can cause nasal congestion and inflammation, leading to Eustachian tube dysfunction.
3. Temporomandibular Joint (TMJ) Disorders: Issues with the jaw joint can cause referred pain to the ear, especially if you grind your teeth or clench your jaw.
4. Earwax Impaction: Excessive earwax can cause pressure and pain in the ear, which may be exacerbated by nasal irrigation.
In conclusion, while your symptoms may improve with home care, persistent issues like ear pain and pressure should not be ignored. An ENT specialist can provide a comprehensive evaluation and recommend appropriate treatment options, which may include medications, further diagnostic tests, or even referral to other specialists if necessary. Taking proactive steps to address your ear health is crucial, especially considering your previous experiences with ear infections and inner ear issues.
