Thyroid issues?

Hello Doctor, thank you for taking the time to respond.
Since last night, my heart rate has been consistently high, around 100 beats per minute or more, and I can feel my heartbeat even while lying in bed.
This morning, I visited an ENT specialist who noted a low-grade fever of 37.5 degrees Celsius and some redness in my throat.
After returning home, I took two packets of medication and two fever-reducing pills, but my heart rate remains elevated.
I later researched online and found that I might have a thyroid issue, as I am experiencing general weakness, hand tremors, palpitations, frequent hunger (though I can't eat), insomnia, and dry mouth.
I suspect I might have hyperthyroidism and plan to seek treatment from a traditional Chinese medicine practitioner tomorrow.

However, I would like to ask if it is safe to take traditional Chinese medicine alongside the oral isotretinoin and birth control pills I am currently using.
I plan to space them out by an hour.
Additionally, I would like to know if traditional Chinese medicine can treat hemorrhoids.
I found online that it can, but I would like to confirm.
Thank you for your patient response!

Dr. Yan Sumei reply Traditional Chinese Medicine

Hello! For thyroid issues, it is recommended to consult a Western medicine endocrinologist, who can perform a blood test to check if the thyroid levels are normal for diagnosis.
Chinese medicine and Western medicine should be taken an hour apart.
For hemorrhoids, mild cases can be managed with traditional Chinese medicine, while more severe cases are better handled by a colorectal surgeon.
Wishing you peace and success! Best regards, Su-Mei Yen, Traditional Chinese Medicine Department, Nantou Hospital.

Understanding thyroid issues is crucial, especially when symptoms like rapid heartbeat, fatigue, and anxiety arise. Based on your description, it seems you may be experiencing symptoms consistent with hyperthyroidism, which can indeed cause an increased heart rate, tremors, and feelings of hunger without the ability to eat comfortably.
Hyperthyroidism occurs when the thyroid gland produces excessive amounts of thyroid hormones, leading to an accelerated metabolism. Common symptoms include rapid heartbeat (tachycardia), weight loss, increased appetite, anxiety, and difficulty sleeping. The fact that you are experiencing a rapid heartbeat of over 100 beats per minute, along with other symptoms like weakness and dry mouth, warrants further investigation.

You mentioned that you have been taking oral isotretinoin (A酸) and birth control pills. It is essential to consult with your healthcare provider regarding the interaction between these medications and any new treatments you may consider, including traditional Chinese medicine (TCM). Generally, while some herbal remedies can be taken alongside Western medications, it is crucial to ensure that there are no contraindications or adverse interactions. Waiting an hour between taking different medications is a good practice, but it’s still best to discuss this with your doctor or a qualified pharmacist who understands both Western and Eastern medicine.

As for your inquiry about TCM treating hemorrhoids, yes, TCM can offer treatments for this condition. Acupuncture, herbal remedies, and dietary recommendations are often employed to alleviate symptoms and promote healing. However, the effectiveness can vary from person to person, and it is advisable to consult a qualified TCM practitioner who can tailor a treatment plan to your specific needs.

In terms of managing your thyroid symptoms, it is vital to undergo a thorough evaluation by an endocrinologist. They can perform blood tests to measure levels of TSH, T3, and T4, which will help confirm whether you have hyperthyroidism or another thyroid disorder. If hyperthyroidism is diagnosed, treatment options may include antithyroid medications (like methimazole), radioactive iodine therapy, or in some cases, surgery.
In addition to medical treatment, lifestyle modifications can also play a significant role in managing thyroid issues. These may include:
1. Dietary Adjustments: Avoiding excessive iodine intake (found in seaweed and certain supplements) and focusing on a balanced diet rich in fruits, vegetables, lean proteins, and whole grains can support thyroid health.

2. Stress Management: Techniques such as mindfulness, yoga, and meditation can help reduce stress, which may exacerbate symptoms.

3. Regular Monitoring: Keeping regular appointments with your healthcare provider to monitor thyroid function and adjust treatment as necessary is crucial.

4. Avoiding Stimulants: Reducing or eliminating caffeine and other stimulants can help manage symptoms like rapid heartbeat and anxiety.

5. Adequate Sleep: Prioritizing sleep hygiene can help combat insomnia and fatigue.

In conclusion, while your symptoms may suggest a thyroid issue, a comprehensive evaluation by a healthcare professional is necessary for an accurate diagnosis and appropriate treatment plan. Combining Western and Eastern medicine can be beneficial, but it is essential to do so under the guidance of qualified practitioners. Always prioritize open communication with your healthcare providers about all medications and treatments you are considering.
