Foreskin issues

Hello Doctor: I am a high school freshman.
My penis can retract the foreskin normally without pain, and I can achieve an erection without issues.
However, during an erection, the foreskin does not retract automatically, and manually retracting it to the coronal sulcus causes pain.
If I try to retract it further, it hurts, and the foreskin seems to be constricting the penis.
I previously saw online that someone used a steroid cream on the foreskin, and after about two weeks, it became normal.
However, the specific application method was not mentioned.
Could you please explain how to apply the medication? For example, should it be applied when soft or hard, should I retract the foreskin to apply it, and if so, how far should I retract it? Thank you, Doctor.

More Info

Phimosis is a condition where the foreskin of the penis cannot be easily retracted over the glans (the head of the penis). This can lead to discomfort, especially during erections, as you've described. It's important to address this condition properly, as it can lead to complications such as infections or inflammation if left untreated.

In your case, it sounds like you have some ability to retract the foreskin when not erect, but experience pain when trying to do so during an erection. This is a common symptom of phimosis, and it can be distressing. The application of topical steroid creams, such as betamethasone or mometasone, is a recognized treatment option for phimosis. These creams can help to soften and stretch the foreskin, making it easier to retract over time.

Application of Steroid Cream for Phimosis
1. Preparation: Before applying the cream, ensure that your hands are clean. Wash the area gently with mild soap and water, and pat it dry with a clean towel.

2. Timing: The cream is typically applied when the penis is flaccid (soft). This is because applying it when erect may cause discomfort and may not allow for even distribution of the cream.

3. Application Technique:
- Retract the Foreskin: If possible, gently retract the foreskin to expose the glans. You should do this carefully to avoid pain. If retracting the foreskin causes significant pain, it may be better to apply the cream without fully retracting it.

- Apply the Cream: Use a small amount of the steroid cream (as directed by your doctor) and apply it to the inner lining of the foreskin and the glans. If you can retract the foreskin, apply it to the area that is tight or causing discomfort. If you cannot retract it fully, apply it to the area that is accessible.

- Massage: Gently massage the cream into the skin. This helps with absorption and may promote stretching of the foreskin.

4. Frequency: Typically, the cream is applied once or twice daily, depending on your doctor's instructions. It's important to follow the prescribed regimen consistently for the best results.

5. Duration: Treatment usually lasts for about 4 to 6 weeks. If you do not see improvement after this period, or if you experience increased pain or other symptoms, you should consult your healthcare provider.

Additional Considerations
- Avoid Irritants: While using the steroid cream, avoid any harsh soaps or products that may irritate the area.

- Monitor for Side Effects: Be aware of potential side effects of steroid creams, such as thinning of the skin or local irritation. If you notice any adverse effects, contact your healthcare provider.

- Follow-Up: Regular follow-up with your healthcare provider is important to monitor progress and make any necessary adjustments to your treatment plan.

When to Seek Further Help
If the steroid cream does not improve your condition or if you experience severe pain, recurrent infections, or other complications, surgical options such as circumcision or preputioplasty may be considered. These procedures can provide a permanent solution to phimosis.

In summary, the application of steroid cream can be an effective treatment for phimosis, but it requires careful application and monitoring. Always consult with your healthcare provider for personalized advice and treatment options tailored to your specific situation.
