Ear pain

I previously visited the doctor several times for outer ear inflammation and was treated with anti-inflammatory medications, antibiotics, topical ointments, and ear drops.
The original inflammation site is now fine, but I still occasionally experience pain and a feeling of blockage in my ear.
I hear popping or cracking sounds when I yawn or swallow.
What could be the cause of this?

Ear pain and discomfort can be quite distressing, especially when it persists despite treatment for conditions like external ear canal inflammation. Based on your description, it seems that you have undergone several evaluations and treatments, yet you continue to experience symptoms such as pain, a feeling of blockage, and unusual sounds when yawning or swallowing.
The symptoms you are experiencing may be indicative of Eustachian tube dysfunction (ETD). The Eustachian tube connects the middle ear to the back of the nose and helps equalize pressure in the ear. When this tube does not function properly, it can lead to a sensation of fullness or pressure in the ear, pain, and even sounds like popping or crackling when you yawn or swallow. This dysfunction can occur due to various reasons, including allergies, sinus infections, or even anatomical variations.

Another possibility is that you may be experiencing residual effects from your previous ear infections or inflammation. Even after the external ear canal inflammation has resolved, the middle ear may still be affected, leading to ongoing discomfort. Conditions like chronic otitis media, which is inflammation of the middle ear, can sometimes persist even when there are no overt signs of infection.

Additionally, the pain and discomfort could also be related to temporomandibular joint (TMJ) dysfunction. The TMJ is located near the ear, and issues with this joint can cause referred pain to the ear area. Symptoms of TMJ dysfunction can include pain when chewing, clicking sounds in the jaw, and discomfort in the ear.

Given your history and the persistence of your symptoms, it would be advisable to seek further evaluation from an otolaryngologist (ENT specialist). They may recommend additional diagnostic tests, such as tympanometry to assess Eustachian tube function, or imaging studies if necessary.
In the meantime, there are some self-care measures you can try to alleviate your symptoms. These include:
1. Nasal Decongestants: If your symptoms are related to nasal congestion, using a decongestant can help open the Eustachian tubes and relieve pressure.

2. Valsalva Maneuver: This technique involves closing your mouth, pinching your nose shut, and gently blowing to equalize pressure in the ears. However, be cautious and do this gently to avoid damaging the eardrum.

3. Warm Compress: Applying a warm compress to the affected ear can help soothe pain and discomfort.

4. Hydration: Staying well-hydrated can help thin mucus and promote better Eustachian tube function.

5. Avoiding Irritants: If allergies are a contributing factor, try to avoid known allergens and consider using antihistamines if appropriate.

6. Soft Diet: If TMJ dysfunction is suspected, eating softer foods can reduce strain on the jaw and alleviate pain.

If your symptoms persist or worsen, or if you experience additional symptoms such as fever, significant hearing loss, or drainage from the ear, it is crucial to seek medical attention promptly. Your goal of becoming an airline pilot is commendable, and addressing these ear issues is essential for your health and safety in that profession.
