Inability to ejaculate after extracorporeal shock wave lithotripsy..?

A few days ago, I went to the emergency room due to severe pain in my right abdomen.
After examination, it was found that I had a ureteral stone blockage.
That day, I underwent shock wave lithotripsy and returned home.
I experienced pain for three to four days, with only the first two days having some stone fragments pass, followed by persistent hematuria.
It seems that I haven't completely cleared the stones, but the pain has lessened.
Afterward, I noticed that I could achieve normal erections with normal rigidity and a sensation of ejaculation, but I found that no semen was produced.
I just had a child, and I'm very worried about what might be wrong.
The day before the severe pain, my sexual function was completely normal, but now suddenly there is no semen or any fluid being released.
What could be happening?

Experiencing dry ejaculation after undergoing shockwave lithotripsy for a ureteral stone can be concerning, especially if your sexual function was normal prior to the procedure. Let's delve into the potential reasons for this phenomenon and what you might consider moving forward.

Understanding Dry Ejaculation:
Dry ejaculation, or the absence of semen during ejaculation, can occur for several reasons. In your case, it’s essential to consider the recent medical intervention you underwent. Shockwave lithotripsy is a non-invasive procedure that uses sound waves to break up kidney stones into smaller pieces, allowing them to pass more easily through the urinary tract. While this procedure is generally safe, it can lead to temporary changes in the urinary and reproductive systems.

Possible Causes of Dry Ejaculation Post-Lithotripsy:
1. Temporary Physical Changes: After the procedure, there may be swelling or irritation in the reproductive tract, which can affect the normal ejaculation process. This could lead to a temporary inability to produce or expel semen.

2. Nerve Function: The nerves that control ejaculation may be affected by the procedure or the pain associated with the kidney stones. If the nerves are temporarily disrupted, this could lead to changes in ejaculation.

3. Medication Effects: If you were prescribed pain medications or any other drugs post-procedure, these could have side effects that impact sexual function and ejaculation. Opioids, for example, can lead to decreased libido and changes in ejaculation.

4. Psychological Factors: The stress and anxiety associated with experiencing severe pain and undergoing a medical procedure can also affect sexual performance and ejaculation. Psychological factors can play a significant role in sexual health.

5. Underlying Conditions: If there were pre-existing conditions affecting your reproductive health, such as retrograde ejaculation (where semen enters the bladder instead of exiting through the penis), these may have been exacerbated by the recent procedure.

What to Do Next:
1. Consult Your Doctor: It’s crucial to discuss your symptoms with your healthcare provider. They can perform a thorough evaluation to determine if there are any underlying issues that need to be addressed. They may also check for any signs of infection or complications from the lithotripsy.

2. Monitor Symptoms: Keep track of any changes in your sexual function, including the return of normal ejaculation, any pain during ejaculation, or changes in libido. This information will be helpful for your doctor.

3. Avoid Self-Diagnosis: While it’s natural to worry about changes in your body, avoid jumping to conclusions about serious conditions without professional guidance. Many post-procedural symptoms are temporary and resolve with time.

4. Consider Psychological Support: If you find that anxiety or stress is affecting your sexual health, consider speaking with a mental health professional. They can help you work through any psychological barriers that may be impacting your sexual function.

5. Follow-Up Care: Ensure you have a follow-up appointment scheduled with your urologist or primary care physician to discuss your recovery and any ongoing symptoms.

In conclusion, while experiencing dry ejaculation after shockwave lithotripsy can be alarming, it is often a temporary condition that can be addressed with proper medical guidance. Open communication with your healthcare provider is key to ensuring a smooth recovery and addressing any concerns you may have regarding your sexual health.
