Inability to ejaculate?
A few days ago, I went to the emergency room due to severe pain in my right abdomen.
After examination, it was found that I had a ureteral stone blockage.
That day, I underwent shock wave lithotripsy treatment and went home.
I experienced pain for three to four days, with only the first two days having some stone fragments pass, followed by continued hematuria.
It seems that I haven't completely cleared the stones, but the pain has subsided.
Afterward, I noticed that I could achieve a normal erection with normal rigidity and a sensation of ejaculation, but I found that no semen was produced.
I just had a child, and I'm very worried about what might be wrong.
The day before the severe pain, my sexual function was completely normal, but now suddenly there is no semen or any fluid being released.
What could be happening?

Experiencing ejaculation issues after undergoing treatment for kidney stones, such as Extracorporeal Shock Wave Lithotripsy (ESWL), can understandably cause concern. It’s important to recognize that the urinary and reproductive systems are closely linked, and any trauma or surgical intervention in the urinary tract can potentially affect sexual function.
Firstly, it’s not uncommon for patients to experience changes in ejaculation following procedures like ESWL. The shock waves used in the treatment can cause some temporary changes in the anatomy and function of the surrounding tissues, including the prostate and seminal vesicles, which are crucial for the production and ejaculation of semen. The presence of blood in the urine post-treatment, as you mentioned, indicates that there may still be some irritation or inflammation in the urinary tract, which can also affect sexual function.
The absence of semen during ejaculation, known as "dry orgasm," can occur for several reasons. One possibility is that the shock waves may have affected the nerves or blood vessels that are responsible for the ejaculation process. Additionally, if there is any swelling or inflammation in the prostate or seminal vesicles, it could impede the normal flow of semen. Another factor to consider is the psychological impact of the pain and discomfort you experienced prior to and following the treatment, which can also influence sexual function.
In your case, since you were able to achieve an erection and felt the sensation of ejaculation, it suggests that the erectile function is intact, but the mechanics of ejaculation may be disrupted. This can sometimes resolve on its own as the body heals and inflammation decreases. However, if the issue persists, it would be advisable to consult with a urologist who can evaluate your condition further. They may recommend tests to assess the prostate and seminal vesicles, and to check for any blockages or other issues that could be contributing to the problem.
In terms of follow-up care, it’s essential to stay hydrated and continue monitoring your symptoms. If you notice any worsening of your condition, such as increased pain, persistent blood in the urine, or any new symptoms, you should seek medical attention promptly.
Lastly, regarding your concerns about the long-term implications of these issues, it’s important to remember that many patients experience temporary changes in sexual function following urological procedures. With time and appropriate medical guidance, many of these issues can resolve.
In summary, while it’s understandable to be worried about the changes you’re experiencing, it’s crucial to give your body some time to heal. If the issue persists, don’t hesitate to reach out to a healthcare professional for further evaluation and support. Your sexual health is an important aspect of your overall well-being, and addressing these concerns with a specialist can help you find the answers and reassurance you need.
