Shortness of breath
Hello Doctor, I am 23 years old, 163 cm tall, and weigh 46 kg.
Since the outbreak of the pandemic, my work has been forced to stop, and I have mostly stayed at home.
Starting last week, I experienced shortness of breath and irregular heartbeats, but it improved after a couple of days.
Then I suddenly noticed many red raised spots on my skin that do not itch, which have persisted until now.
Over the past few days, I have also felt dizzy and a sensation of pressure in my head.
Two days ago, I started to have phlegm, and yesterday I suddenly found it difficult to breathe normally while lying down, feeling a blockage in my throat and an increased pressure in my head, while my nasal passages are clear without any mucus obstruction.
I would like to know if this is related to issues with my lungs or respiratory system, or if it could be a heart problem, or if it is related to my lack of exercise from staying at home for a long time, or possibly due to psychological factors and excessive stress...
Thank you.

Shortness of breath, or dyspnea, can be a concerning symptom, especially for young adults like yourself. Given your age and the symptoms you've described, it's essential to consider a variety of potential causes, both physical and psychological.
Firstly, your recent history of being largely sedentary due to the pandemic could contribute significantly to your current symptoms. Lack of physical activity can lead to deconditioning, which may manifest as shortness of breath during normal activities. When the body is not accustomed to regular exercise, even minor exertion can feel overwhelming, leading to feelings of breathlessness.
Additionally, the symptoms you mentioned, such as irregular heartbeats (palpitations), dizziness, and the sensation of pressure in your head, could indicate a few different issues. While it’s reassuring that these symptoms have fluctuated, they warrant further investigation. The presence of red spots on your skin, which are not itchy, could suggest an allergic reaction or another dermatological issue that may or may not be related to your respiratory symptoms.
The feeling of a blocked throat and difficulty breathing while lying down could indicate several possibilities. One common issue is post-nasal drip, which can occur due to allergies or sinusitis, leading to mucus accumulation in the throat and causing a sensation of obstruction. This can also contribute to coughing up phlegm, as you noted. If you have a history of allergies or respiratory issues, this could be a significant factor.
Moreover, anxiety and stress can exacerbate feelings of breathlessness. The pandemic has caused widespread anxiety, and many individuals experience heightened stress responses, which can manifest physically. Even if you don’t consciously feel anxious, your body might be reacting to stressors in your environment, leading to symptoms like shortness of breath, palpitations, and dizziness.
Given the complexity of your symptoms, it is crucial to consult with a healthcare professional who can perform a thorough evaluation. This may include a physical examination, pulmonary function tests, and possibly imaging studies like a chest X-ray or CT scan to assess your lungs and heart. Blood tests may also be necessary to rule out infections or other systemic issues.
In the meantime, consider implementing some lifestyle changes that could help alleviate your symptoms. Gradually increasing your physical activity can improve your cardiovascular fitness and respiratory function. Simple exercises, such as walking or stretching, can be beneficial. Additionally, practicing relaxation techniques, such as deep breathing exercises, meditation, or yoga, may help manage stress and anxiety, potentially reducing the sensation of breathlessness.
Lastly, if you notice any worsening of your symptoms, such as increased difficulty breathing, chest pain, or persistent dizziness, seek immediate medical attention. Your health and well-being are paramount, and addressing these symptoms early can lead to better outcomes.
