Glans issues and lip issues?

Hello, Doctor.
I often experience sudden redness and swelling around my lips every one to two years, sometimes accompanied by peeling and bleeding.
This condition also affects my glans.
On one occasion, I visited a urology and dermatology specialist, who suggested it might be a drug allergy, but I haven't taken any medication.
It tends to recur after a while, which is really troubling.
I have a long-standing issue with scrotal eczema that hasn't been resolved, and I frequently experience swollen and painful gums (I have had root canal treatment for cavities).
Could these conditions be related? Each time the affected area occurs, I apply erythromycin ointment for about 3 to 5 days, and it gradually improves.
What should I do to resolve this issue, and which specialty should I consult? Thank you for your guidance!

Understanding lip and glans issues can be complex, especially when symptoms like redness, swelling, peeling, and bleeding occur intermittently. Based on your description, it seems you are experiencing recurrent episodes that may be linked to various underlying causes, including allergic reactions, dermatological conditions, or even systemic issues.

Possible Causes
1. Allergic Reactions: Although you mentioned not taking any medication, allergic reactions can still occur due to environmental factors, food, or contact allergens. For instance, certain lip balms, cosmetics, or even dental products can trigger contact dermatitis, leading to symptoms on the lips and glans.

2. Dermatitis: Conditions like eczema or seborrheic dermatitis can affect both the lips and the glans. Your history of chronic scrotal eczema suggests a predisposition to skin conditions that may flare up under certain conditions, such as stress, weather changes, or irritants.

3. Infections: Fungal infections, particularly candidiasis, can cause similar symptoms on the glans. If the skin barrier is compromised, it may lead to secondary infections, which could explain the recurrent nature of your symptoms.

4. Systemic Conditions: Conditions like psoriasis or autoimmune disorders can manifest with skin symptoms in various locations, including the lips and genital area. Your recurrent gingival swelling may also suggest a systemic inflammatory process.

5. Oral Health Issues: The mention of painful gums and a history of dental issues could indicate that oral health plays a role in your symptoms. Infections or inflammation in the mouth can sometimes correlate with skin issues.

Treatment Options
1. Topical Treatments: You mentioned using erythromycin ointment, which is an antibiotic that can help with secondary infections. However, if the underlying cause is allergic or inflammatory, a topical corticosteroid may be more effective in reducing inflammation and symptoms.

2. Avoiding Irritants: Identifying and avoiding potential allergens or irritants is crucial. This may involve switching to hypoallergenic products for your lips and avoiding known irritants in your environment.

3. Moisturization: Keeping the affected areas well-moisturized can help prevent flare-ups, especially in dry conditions. Look for fragrance-free and hypoallergenic moisturizers.

4. Consulting Specialists: Given the complexity of your symptoms, it would be beneficial to consult a dermatologist for a thorough evaluation. They can perform patch testing to identify potential allergens and recommend appropriate treatments. If oral health issues are significant, a visit to a dentist or an oral surgeon may also be warranted.

5. Systemic Evaluation: If symptoms persist despite topical treatments, consider a systemic evaluation to rule out underlying conditions. Blood tests or referrals to specialists in immunology or rheumatology may be necessary.

Conclusion
Your recurrent symptoms on the lips and glans, along with the history of eczema and dental issues, suggest a multifactorial problem that may require a comprehensive approach to diagnosis and treatment. It is essential to work closely with healthcare providers to identify the underlying causes and develop an effective management plan. Keeping a symptom diary, noting any triggers or patterns, can also be helpful for your healthcare provider in diagnosing your condition.
