Weight Gain Concerns with Sertraline During Pregnancy: What to Know - Psychiatry

Issues related to weight gain associated with Seroquel?

Hello, Doctor! I am a 36-week pregnant woman.
I have been taking psychiatric medications for many years before pregnancy and started to reduce my dosage after becoming pregnant.
In the mid to late stages of my pregnancy, I began taking Sertraline, currently at a dosage of approximately 25 mg twice a day.
Fortunately, I have not experienced prenatal depression or insomnia, and my entire pregnancy has been stable with a regular routine.

However, I am very concerned about my weight, especially since I gained four kilograms this month.
I have heard that many people gain weight while taking Sertraline, but pregnancy itself can also lead to weight gain, making it difficult to determine the cause of my weight increase.
My doctor believes that the dosage I am using is not high and should not be causing this level of weight gain.

Could you please clarify if this dosage is indeed unlikely to cause such significant weight gain? Additionally, is weight gain from this type of medication typically associated with increased appetite, or can one gain weight even with a normal food intake? Is it related to a decrease in metabolism or something similar? Thank you!

Hello: Weight gain from taking Seroquel varies from person to person.
For some, it is quite noticeable, while for others it is less so, usually due to increased appetite.
Generally, the higher the dosage, the more pronounced the effect.
A dosage of 50 mg is not considered high.
Thank you for your message.

Weight gain during pregnancy can be a significant concern for many expectant mothers, especially when they are also taking medications like Sertraline (known as Zoloft). It’s important to understand the potential effects of this medication on weight and how it interacts with the physiological changes that occur during pregnancy.

Sertraline is a selective serotonin reuptake inhibitor (SSRI) commonly prescribed for depression and anxiety disorders. While it is classified as a Category C medication during pregnancy, meaning that risk cannot be ruled out, many women continue to take it to manage their mental health. The benefits of treating depression during pregnancy often outweigh the potential risks to the fetus, as untreated depression can lead to adverse outcomes for both the mother and the baby.

Regarding your specific concerns about weight gain while taking Sertraline, it is essential to recognize that weight gain during pregnancy is influenced by multiple factors, including hormonal changes, dietary habits, and physical activity levels. SSRIs, including Sertraline, can affect weight in different ways. Some patients report weight gain, while others may experience weight loss or no significant change. The mechanism behind weight gain with SSRIs is not entirely understood, but it may involve increased appetite, changes in metabolism, or fluid retention.

In your case, the increase of four kilograms in one month could be attributed to several factors. Firstly, pregnancy itself is associated with weight gain due to the growing fetus, increased blood volume, and changes in body composition. The recommended weight gain during pregnancy varies based on pre-pregnancy weight; for women with a normal BMI, the guideline is typically around 25-35 pounds (approximately 11-16 kg) for the entire pregnancy. If you started your pregnancy at a higher weight, your doctor might recommend a lower total weight gain.

As for the dosage of Sertraline you are taking (25 mg twice daily), this is generally considered a low to moderate dose. While some individuals may experience weight gain at higher doses, it is less likely that your current dosage is the primary cause of your weight gain. However, if you notice an increase in appetite or cravings, this could be a side effect of the medication. It’s also worth noting that some women may experience changes in their metabolism during pregnancy, which can affect how their bodies process food and energy.

If you are concerned about your weight gain, it would be beneficial to discuss this with your healthcare provider. They can help you assess whether your weight gain is within the expected range for your stage of pregnancy and whether any adjustments to your diet or activity level are necessary. Additionally, they can evaluate your mental health needs and determine if any changes to your medication regimen are appropriate.

In summary, while Sertraline can potentially contribute to weight gain, it is essential to consider the broader context of pregnancy-related changes. Monitoring your weight, maintaining a balanced diet, and staying active can help manage weight gain during pregnancy. Always consult with your healthcare provider before making any changes to your medication or lifestyle to ensure the best outcomes for both you and your baby.
