Oral issues

Hello Doctor, I have previously inquired about a severe sensation of a foreign body in my throat, but I do not experience difficulty swallowing; the sensation just persists.
I used my hand to feel the back of my tongue near the throat and noticed many protrusions at the base of my tongue, which feel somewhat soft and firm.
I had mentioned before that I underwent an endoscopy during a follow-up visit, but I am unsure if I communicated any issues regarding this area.
Typically, when doctors examine the throat, can they also see this? I am now wondering if the sensation of a foreign body in my throat could be influenced by the protrusions at the base of my tongue.
Additionally, I have pain on the left side where my tonsil was removed, and I have been experiencing unilateral tinnitus for over two months, which has not subsided.
I am very worried that these symptoms could indicate a serious illness.
Could you please help analyze whether I should seek further examination from a doctor? Is it possible to see any issues in the oral cavity, throat, and tongue during an endoscopy? Are the protrusions at the base of the tongue normal? If there were any lumps, would they be easy to detect? I have many questions, and I appreciate your response.

Hello: The protrusion at the base of the tongue may be a circumvallate papilla, which can generally be observed during an endoscopic examination.
If the clinician does not find any significant issues, it is mostly normal tissue of the tongue base.
If you are concerned, you may visit an otolaryngologist for further evaluation.
Wishing you peace and health.
Sincerely, Dr.

More Info

Throat discomfort can be a common yet concerning symptom that many individuals experience. It can manifest in various ways, such as a sensation of a lump or foreign body in the throat, pain, or difficulty swallowing. Understanding the potential causes of throat discomfort and knowing when to seek medical help is crucial for effective management and peace of mind.

Common Causes of Throat Discomfort
1. Allergies: Allergic reactions to pollen, dust, or pet dander can lead to throat irritation and discomfort. This is often accompanied by other symptoms such as sneezing, nasal congestion, or itchy eyes.

2. Infections: Viral infections, such as the common cold or flu, can cause inflammation in the throat, leading to discomfort. Bacterial infections, like strep throat, can also result in severe throat pain and may require antibiotic treatment.

3. Gastroesophageal Reflux Disease (GERD): Acid reflux can cause stomach acid to flow back into the esophagus, leading to a burning sensation in the throat and a feeling of discomfort.

4. Tonsillitis: Inflammation of the tonsils can cause pain, especially when swallowing. This condition may also present with fever and swollen lymph nodes.

5. Throat Irritation: Smoking, exposure to secondhand smoke, or environmental irritants can lead to throat discomfort. Dry air, especially in winter months, can also contribute to throat irritation.

6. Muscle Strain: Overuse of the throat muscles, such as excessive talking or yelling, can lead to soreness and discomfort.

7. Growths or Lesions: In some cases, abnormal growths or lesions in the throat can cause discomfort. These may be benign or malignant, and any persistent changes should be evaluated by a healthcare professional.

When to Seek Help
It is essential to seek medical attention if you experience any of the following symptoms:
- Persistent throat discomfort lasting more than a week.

- Difficulty swallowing or breathing.

- Severe pain that does not improve with over-the-counter medications.

- Presence of a lump or swelling in the throat.

- Accompanying symptoms such as high fever, rash, or joint pain.

- Unexplained weight loss or changes in voice.

Specific Concerns Regarding Your Symptoms
Based on your description, it seems you are experiencing a persistent sensation of a foreign body in your throat, pain on one side near the tonsil area, and unilateral ear ringing (tinnitus). These symptoms can be concerning, especially if they have persisted for over two months. The presence of "bump-like" structures at the base of your tongue (the tongue root) can be normal, as this area contains lymphoid tissue known as lingual tonsils, which can appear prominent in some individuals.

However, the combination of your symptoms—especially the unilateral ear ringing and pain—warrants further investigation. While an endoscopy can provide a thorough examination of the throat and surrounding structures, it is crucial to communicate all your symptoms clearly to your healthcare provider. They may consider additional imaging studies, such as a CT scan, to rule out any serious underlying conditions.

Conclusion
In summary, throat discomfort can arise from various causes, and while many are benign, persistent or severe symptoms should not be ignored. Given your ongoing symptoms, it is advisable to return to your healthcare provider for a comprehensive evaluation. Be sure to discuss all your concerns, including the sensation of a lump, pain, and ear symptoms, to ensure a thorough assessment. Early intervention can lead to better outcomes and alleviate your worries about potential serious conditions.
