Darkening and hyperpigmentation around the lips?
Hello Doctor, my inner lip area is pink, but the pigmentation around my lips and at the corners has become very severe, and even the philtrum area is starting to darken.
I initially thought it might be due to not properly removing my makeup for a while, which could have caused the pigmentation.
However, I have been diligent about removing my makeup recently, yet the pigmentation has worsened.
Recently, even the area above my lips, close to the philtrum, has started to darken (not due to shadow from fine hairs).
From a Traditional Chinese Medicine perspective, it might be related to spleen and stomach deficiency with cold.
I have consulted a gastroenterologist, and although I have symptoms of a gastric ulcer, the doctor noted that my palms do not show cyanosis, suggesting that the darkening of my lips is likely a dermatological issue.
Therefore, I visited a dermatologist, who recommended that I undergo blood tests at a hospital to check for any abnormal release of melanin from the brain.
Since there are no large medical facilities nearby for blood tests, and given the severity of the pandemic, I am hesitant to travel north for treatment.
I would like to ask you first: what conditions could be indicated by the results of the blood tests? Based on the findings, how can I improve my condition? This issue has been troubling me for a long time, and my friends and family have even asked if I am getting sick (crying with my face covered).
I would greatly appreciate your response.

Excessive makeup removal and friction may be the cause.
Without seeing the lesions, it's difficult to determine the exact issue.
If it is very bothersome, please seek medical attention promptly.

More Info
The darkening around the lips, particularly the pigmentation changes you are experiencing, can be attributed to several factors, including environmental influences, lifestyle choices, and underlying health conditions. Let's explore the potential causes and solutions for this issue.
Causes of Darkening Around the Lips
1. Hyperpigmentation: This is the most common cause of dark patches on the skin, including around the lips. Hyperpigmentation can result from prolonged sun exposure, hormonal changes, or post-inflammatory changes from skin irritation or trauma.
2. Skin Irritation: If you have not been diligent in removing makeup or if you have been using products that irritate your skin, this can lead to darkening. Ingredients in cosmetics, especially those that are not suited for your skin type, can cause irritation and subsequent pigmentation.
3. Diet and Nutrition: Traditional Chinese medicine often links skin health to internal organ function. A deficiency in certain nutrients, particularly vitamins A, C, and E, can affect skin health. Additionally, digestive issues, such as those related to the spleen and stomach, may manifest as skin problems.
4. Underlying Medical Conditions: Conditions such as liver disease, hormonal imbalances, or even certain medications can lead to changes in pigmentation. If your gastrointestinal issues, like ulcers, are affecting your overall health, they could contribute to skin changes.
5. Genetics: Some individuals are more prone to pigmentation changes due to their genetic makeup. If there is a family history of similar skin issues, this could be a contributing factor.
Diagnostic Approach
Since you have already consulted with a gastroenterologist and a dermatologist, it is wise to follow their advice regarding blood tests. Blood tests can help rule out conditions such as liver dysfunction, hormonal imbalances, or deficiencies in vitamins and minerals.
1. Liver Function Tests: These can help assess if your liver is functioning properly, as liver health is closely tied to skin appearance.
2. Hormonal Panels: If hormonal imbalances are suspected, tests for thyroid function or sex hormones may be warranted.
3. Nutritional Assessments: Checking levels of vitamins and minerals can help identify deficiencies that may be affecting your skin.
Treatment Options
1. Topical Treatments: Depending on the diagnosis, your dermatologist may recommend topical treatments such as hydroquinone, retinoids, or vitamin C serums to help lighten hyperpigmentation. Sunscreen is crucial to prevent further darkening.
2. Chemical Peels or Laser Treatments: For more persistent pigmentation, dermatologists may suggest chemical peels or laser treatments to remove the top layers of skin and promote new skin growth.
3. Lifestyle Modifications: Ensure you are maintaining a balanced diet rich in fruits, vegetables, and whole grains. Staying hydrated and reducing sun exposure can also help improve skin health.
4. Regular Follow-ups: Keep in touch with your healthcare providers to monitor your condition and adjust treatments as necessary.
Conclusion
The darkening around your lips can be multifactorial, and while it may be distressing, understanding the underlying causes is the first step toward effective treatment. Blood tests can provide valuable insights into your health, and addressing any deficiencies or imbalances may significantly improve your skin condition. Remember to be patient, as changes may take time, and maintaining a consistent skincare routine is essential. If you have further concerns or if your condition worsens, do not hesitate to seek medical advice.
