Persistent Nasal Congestion After Cold: Allergic Rhinitis - Otolaryngology

Allergic nasal congestion caused by a cold that cannot be resolved?

Hello Doctor: About a month ago, I had a cold and visited an otolaryngologist.
The doctor mentioned that the cold triggered nasal allergies.
While the other symptoms of the cold have resolved, I still have persistent nasal congestion.
In the morning, one side of my nose is completely blocked, and during the day, it tends to be congested on one side but not completely blocked.
At work, I experience a runny nose and post-nasal drip, possibly due to the strong air conditioning.
I have tried medication before, but it hasn't helped.
I will be changing environments in two months; can I temporarily ignore these allergies? Additionally, when I clean out the mucus from my left nostril, I notice a sensitive area that tends to bleed a little when I dig in.
Most of my nasal congestion is on the left side, although it occasionally shifts to the right.
I'm wondering if there is any connection between these symptoms.

Hello: Common allergies often cause sneezing and a runny nose, which can also lead to nasal congestion.
If you have used medication without significant improvement and are experiencing persistent unilateral nasal congestion, it is advisable to return for an examination to determine if it is related to a deviated septum or the presence of a unilateral nasal polyp.
Wishing you safety and health.
Sincerely, Dr.

Persistent nasal congestion following a cold can often be attributed to allergic rhinitis, especially if the symptoms linger after other cold symptoms have resolved. Allergic rhinitis is an inflammatory condition of the nasal mucosa caused by allergens, which can lead to symptoms such as nasal congestion, runny nose, sneezing, and postnasal drip.
In your case, it seems that after experiencing a cold, you have developed ongoing nasal congestion that primarily affects one side of your nose, with occasional shifts to the other side. This pattern can be indicative of several underlying issues.

Possible Causes of Your Symptoms:
1. Post-Viral Rhinitis: After a viral infection like a cold, the nasal passages can remain inflamed and sensitive, leading to prolonged symptoms even after the initial infection has resolved. This is sometimes referred to as post-viral rhinitis.

2. Allergic Rhinitis: If you notice that your symptoms worsen in certain environments, such as in your office with strong air conditioning, it could suggest an allergic component. Common allergens include dust mites, pollen, mold, and pet dander. The nasal congestion you experience, especially if it alternates sides, can be a sign of allergic rhinitis.

3. Nasal Polyps or Structural Issues: If you have a history of nasal issues, such as previous surgeries or chronic sinusitis, structural abnormalities like nasal polyps or a deviated septum could contribute to persistent congestion. These conditions can obstruct airflow and lead to one-sided nasal blockage.

4. Dry Air: Air conditioning can dry out the nasal mucosa, leading to irritation and increased susceptibility to bleeding, which might explain the nosebleeds you experience when cleaning your nostrils.

Recommendations for Management:
1. Avoid Irritants: Try to identify and avoid potential allergens or irritants in your environment. This may include using air purifiers, keeping windows closed during high pollen seasons, and regularly cleaning your living space to reduce dust and allergens.

2. Saline Nasal Irrigation: Using a saline nasal spray or performing nasal irrigation can help moisten the nasal passages, clear out mucus, and reduce congestion. This can be particularly helpful if you are experiencing postnasal drip.

3. Over-the-Counter Medications: Antihistamines can be effective in managing allergic rhinitis symptoms. Nasal corticosteroids are also highly effective for reducing inflammation in the nasal passages and can help alleviate congestion. However, these should be used under the guidance of a healthcare provider.

4. Monitor Symptoms: Keep track of your symptoms and any potential triggers. If your nasal congestion persists or worsens, it may be beneficial to consult with an allergist or an ear, nose, and throat (ENT) specialist for further evaluation. They may recommend allergy testing or imaging studies to assess for structural issues.

5. Avoid Nose Picking: The presence of blood when you clean your nose suggests that the nasal mucosa may be fragile or irritated. Avoid picking your nose, as this can exacerbate bleeding and irritation.

When to Seek Further Medical Attention:
If your symptoms do not improve with over-the-counter treatments or if you experience severe symptoms such as significant facial pain, persistent nosebleeds, or changes in vision, it is important to seek medical attention. An ENT specialist can provide a thorough examination and may recommend treatments such as allergy testing or imaging studies to rule out any underlying conditions.

In summary, while it may be tempting to let your symptoms go untreated, addressing persistent nasal congestion is important for your overall comfort and health. With appropriate management and lifestyle adjustments, you can significantly improve your quality of life.
