Numbness in the hands?

I apologize for bothering you, doctor.
Some time ago, I spent a long time in bed using my phone and computer, and I have been experiencing pain in my left shoulder and neck that radiates down to my ring and little fingers, which sometimes feel numb.
Occasionally, my legs also feel numb, and I experience dizziness at times.
After practicing yoga for a while, I felt some improvement.
However, one day I noticed that my scalp also felt numb.
I consulted a doctor who mentioned that there might be some compression in my cervical spine.
This afternoon, after taking a nap (possibly sleeping on my stomach), I woke up with numbness on the palm side of my right hand and little finger, and my left knee also feels slightly numb.
I would like to ask the doctor about the possible causes and whether it could be amyotrophic lateral sclerosis (ALS).
Thank you!

Niming, 20~29 year old female. Ask Date: 2021/07/10

Dr. Cai Zongzhang reply Neurology

Hello! Based on the above, these symptoms do not match the clinical characteristics of amyotrophic lateral sclerosis (ALS).
It is recommended to visit a neurology outpatient clinic.

More Info

Understanding hand numbness and associated symptoms can be quite concerning, especially when they persist or worsen over time. Based on your description, it seems that you are experiencing a range of symptoms, including numbness in your left hand's ring and little fingers, as well as occasional numbness in your legs and head. You also mentioned experiencing neck pain, which has been linked to cervical spine issues. Let's break down the potential causes and concerns regarding your symptoms.

Potential Causes of Numbness
1. Cervical Radiculopathy: Given your history of neck pain and the numbness radiating down your arm, it is possible that you are experiencing cervical radiculopathy. This condition occurs when a nerve in the neck is compressed or irritated, often due to herniated discs or degenerative changes in the cervical spine. This can lead to symptoms such as pain, numbness, and tingling in the arms and hands.

2. Ulnar Nerve Compression: The numbness in your little finger and ring finger could indicate ulnar nerve compression, commonly known as cubital tunnel syndrome. This occurs when the ulnar nerve, which runs along the inner side of the elbow, becomes compressed. Symptoms may include numbness, tingling, and weakness in the hand.

3. Poor Posture: Spending long periods in a poor posture, especially while using a phone or computer, can lead to muscle tension and nerve compression. This can cause symptoms like numbness and tingling in the extremities.
4. Peripheral Neuropathy: This condition involves damage to the peripheral nerves and can result from various factors, including diabetes, vitamin deficiencies, or alcohol use. Symptoms typically include numbness, tingling, and weakness in the hands and feet.

5. Anxiety and Stress: Psychological factors can also contribute to physical symptoms. Anxiety can lead to muscle tension, which may exacerbate nerve compression and result in numbness or tingling sensations.

Concerns Regarding ALS (Amyotrophic Lateral Sclerosis)
You expressed concern about the possibility of ALS, commonly known as Lou Gehrig's disease. While it is understandable to worry about serious conditions, it is important to note that ALS typically presents with progressive muscle weakness, atrophy, and fasciculations (muscle twitches). Numbness and tingling are not common early symptoms of ALS. If you are experiencing significant muscle weakness or atrophy, it would be prudent to consult a neurologist for a thorough evaluation.

Recommendations for Management
1. Posture Correction: Focus on maintaining good posture while using electronic devices. Ergonomic adjustments can help reduce strain on your neck and shoulders.

2. Physical Therapy: Engaging in physical therapy can help strengthen the muscles around your neck and improve flexibility. A therapist can also provide exercises to alleviate nerve compression.

3. Yoga and Stretching: As you mentioned that yoga has helped, continuing with gentle stretching and yoga can improve your overall flexibility and reduce muscle tension.

4. Medical Evaluation: Since you have experienced persistent symptoms, it is advisable to seek a comprehensive evaluation from a healthcare professional. This may include imaging studies like an MRI of the cervical spine to assess for any structural issues.

5. Lifestyle Modifications: Ensure you are maintaining a balanced diet, staying hydrated, and managing stress through relaxation techniques or mindfulness practices.

Conclusion
In summary, while your symptoms may be concerning, they are more likely related to issues such as cervical radiculopathy or nerve compression rather than a serious condition like ALS. It is essential to address your symptoms with appropriate medical care and lifestyle modifications. If symptoms persist or worsen, do not hesitate to seek further evaluation from a healthcare provider. Your health and well-being are paramount, and taking proactive steps can lead to significant improvements.
