Glans issues

Hello, doctor.
I have a few questions.
I have a problem with phimosis, but it does not affect ejaculation or urination.
Normally, the glans cannot be exposed, but it can be slightly exposed during an erection.
When I masturbate, I feel that the glans is tightly constricted and seems unable to come out; when it does come out, it is not fully exposed and is instead trapped by the foreskin, causing pain.
Sometimes, when I touch the glans with my hand, I experience a slight stinging sensation.
Could this be due to inadequate cleaning of the glans? Is there a necessity for circumcision given the above conditions?

Dr. Du Shixi reply Urology

Hello: If the foreskin cannot be fully retracted during an erection and the glans is exposed, the doctor may recommend surgery.
It is advisable to have a medical examination.
However, in some cases, the tightness may be mild and could gradually improve.
For such foreskin conditions, it is important to avoid prolonged retraction of the foreskin, as this can lead to edema.
Wishing you good health.

More Info

Phimosis is a condition characterized by the inability to retract the foreskin over the glans (the head of the penis). It can be a source of discomfort and concern for many men, especially when it leads to symptoms such as pain, difficulty during sexual activity, or hygiene issues. Based on your description, it seems you are experiencing some discomfort and tightness associated with your foreskin, particularly during masturbation and when attempting to expose the glans.

Symptoms of Phimosis
The symptoms you mentioned, such as the inability to fully retract the foreskin, tightness, and pain during sexual activities, are common in cases of phimosis. The slight pain or discomfort when touching the glans could be due to a combination of factors, including:
1. Sensitivity of the Glans: If the glans has not been exposed regularly, it may be more sensitive, leading to discomfort when touched.

2. Hygiene Issues: If the area is not cleaned properly, smegma (a combination of dead skin cells and oils) can accumulate under the foreskin, potentially leading to irritation or infection.

3. Tight Foreskin: The tightness of the foreskin can cause pain during erections or sexual activity, as you described.

Concerns Related to Phimosis
While phimosis itself is not always a serious medical condition, it can lead to complications if left untreated. Some potential concerns include:
- Infections: Difficulty in cleaning the area can lead to infections, such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin).

- Pain during Sexual Activity: As you noted, the tightness can lead to pain during masturbation or intercourse, which can affect sexual health and enjoyment.

- Psychological Impact: Concerns about phimosis can lead to anxiety or embarrassment, affecting self-esteem and relationships.

Treatment Options
1. Conservative Management: In some cases, gentle stretching exercises of the foreskin may help alleviate tightness. This should be done carefully and gradually to avoid pain or injury. Topical corticosteroids may also be prescribed to help reduce inflammation and facilitate stretching.

2. Circumcision: If conservative measures are ineffective or if you experience significant discomfort, circumcision (surgical removal of the foreskin) may be recommended. This procedure can provide a permanent solution to phimosis and eliminate the associated symptoms.

3. Preputioplasty: This is a less invasive surgical option where the foreskin is surgically widened without complete removal. This may be suitable for some individuals who wish to retain their foreskin but alleviate the tightness.

Conclusion
Given your symptoms and the discomfort you are experiencing, it may be beneficial to consult with a urologist or a healthcare provider specializing in male reproductive health. They can perform a thorough examination, discuss your symptoms in detail, and help you determine the most appropriate treatment option based on your specific situation.
In the meantime, maintaining good hygiene by gently cleaning the area can help reduce the risk of infections and irritation. If you have any concerns about your symptoms or if they worsen, do not hesitate to seek medical advice. Remember, addressing these issues early can lead to better outcomes and improved quality of life.
