Menstrual irregularities
Dear Dr.
Cheng: My menstrual cycle has generally been regular, about 30 or 31 days.
In December: I started on December 24, 1991; in January: I started on January 24, 1992; on February 2, I took emergency contraceptive pills; in February: I started on February 14, with normal flow.
As of now, I have not had my period since April 22.
On April 4, I visited a gynecologist, who said I was not pregnant (urine test + ultrasound).
On April 14, I saw another gynecologist, who also confirmed I was not pregnant (urine test + ultrasound) and prescribed medication for 3 days.
The doctor mentioned that my period should return 2 days after stopping the medication, but I finished the medication on April 17 as scheduled, and I still have not gotten my period.
What could be the reason for my continued absence of menstruation? Is it true, as the doctor said, that "it's okay if it doesn't come occasionally"? Hormonal imbalance was mentioned by both doctors—if that is the case, how should I supplement my diet? Are there any precautions I should take to avoid blockages or other adverse conditions, including tumors?

Hello: There are many reasons for a missed period, such as approaching menopause, ovarian disorders or degeneration, underdevelopment, hormonal imbalances due to stress, elevated prolactin levels, etc.
Of course, pregnancy is the most likely cause.
Therefore, you may wait another two weeks before visiting the clinic for follow-up.
If your periods are usually regular and suddenly stop, you should see a doctor to rule out pregnancy or any pathological conditions (it is essential to exclude the aforementioned situations).
Dietary supplements cannot change the status of menstruation.

Irregular menstruation can be a source of concern for many women, especially when their cycles have been regular in the past. Understanding the causes, potential concerns, and remedies for irregular menstruation is crucial for managing reproductive health effectively.
Causes of Irregular Menstruation
1. Hormonal Imbalances: The menstrual cycle is regulated by hormones, and any disruption in hormone levels can lead to irregular periods. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, and adrenal gland disorders can cause hormonal imbalances.
2. Stress: High levels of stress can affect the hypothalamus, which regulates the hormones that control the menstrual cycle. Emotional or physical stress can lead to missed or delayed periods.
3. Weight Changes: Significant weight loss or gain can impact hormone levels. Low body weight, often seen in eating disorders, can lead to amenorrhea (absence of menstruation), while obesity can lead to irregular cycles due to increased estrogen levels.
4. Medications: Certain medications, including hormonal contraceptives and some mental health medications, can affect menstrual cycles. The use of emergency contraception, as mentioned in your case, can also lead to temporary changes in your cycle.
5. Underlying Health Conditions: Conditions such as diabetes, liver disease, and certain cancers can affect menstrual regularity. It’s essential to rule out these conditions if irregularities persist.
Concerns Related to Irregular Menstruation
1. Pregnancy: As you have already undergone tests to confirm that you are not pregnant, this concern may be alleviated. However, if periods remain absent, further investigation may be warranted.
2. Hormonal Disorders: Persistent irregularities can indicate underlying hormonal issues that may require treatment. Conditions like PCOS can lead to long-term health issues if not managed properly.
3. Potential for Other Health Issues: Irregular menstruation can sometimes be a symptom of more severe health problems, including uterine fibroids or endometriosis. If left unchecked, these conditions can lead to complications.
4. Emotional Impact: Irregular periods can cause anxiety and stress, which can further exacerbate the problem. It’s essential to address emotional well-being alongside physical health.
Remedies and Management
1. Diet and Nutrition: A balanced diet rich in whole foods, including fruits, vegetables, lean proteins, and healthy fats, can help regulate hormones. Specific nutrients, such as omega-3 fatty acids, vitamin D, and magnesium, may support hormonal balance. Foods rich in fiber can also help maintain a healthy weight.
2. Regular Exercise: Engaging in regular physical activity can help manage weight and reduce stress, both of which can contribute to more regular menstrual cycles.
3. Stress Management: Techniques such as yoga, meditation, and mindfulness can help reduce stress levels, potentially leading to more regular cycles.
4. Medical Consultation: If irregular periods persist, it’s essential to consult with a healthcare provider. They may recommend blood tests to check hormone levels or imaging studies to evaluate the reproductive organs.
5. Hormonal Treatments: In some cases, hormonal treatments may be necessary to regulate the menstrual cycle. This could include birth control pills or other hormonal therapies.
6. Monitoring: Keeping a menstrual diary can help track cycles and identify patterns or triggers for irregularities. This information can be valuable for healthcare providers in diagnosing and treating the issue.
Conclusion
Irregular menstruation can be caused by various factors, including hormonal imbalances, stress, and lifestyle changes. While occasional irregularities may not be a cause for concern, persistent issues warrant further investigation. It’s essential to maintain open communication with your healthcare provider and consider lifestyle modifications that promote hormonal balance. If you have ongoing concerns, seeking a second opinion or further testing may provide additional insights into your reproductive health.
