Hemorrhoid surgery issues?
I previously experienced significant bleeding (similar to heavy menstrual flow) every time I used the restroom due to my hemorrhoids, and I needed to manually push them back in.
I had originally scheduled a rubber band ligation procedure, but during a colonoscopy, some white spots were discovered on the hemorrhoids.
Therefore, the doctor recommended surgery to remove them for biopsy.
Both doctors have suggested this, so I plan to proceed with the surgery, but I have some questions regarding the surgical options.
1.
The doctor recommended traditional surgery (with out-of-pocket costs for the ultrasound knife) to remove the largest hemorrhoid with the white spots for biopsy, while leaving the two smaller internal hemorrhoids (approximately stage II) intact.
Will these smaller hemorrhoids worsen in the future and require surgery? Alternatively, can they be treated with rubber band ligation later on? Is it less damaging to the anal area to only remove the larger hemorrhoid and leave the smaller ones, potentially reducing the risk of complications? Or would it be better to address all of them at once?
2.
The doctor mentioned that if I want to remove the smaller hemorrhoids, they could be treated with laser due to my insurance coverage.
This method would only leave a small incision, with minimal bleeding and pain.
However, I found online that laser treatment seems to be an outdated technique that is rarely used.
Am I misinformed, or are there different types of lasers? I would like to know if this method has many complications, or if it would be better to remove everything using the ultrasound knife.
I realize these questions are quite complex, and I appreciate the doctor's time in addressing them.

When considering treatment options for hemorrhoids, particularly in your case where there is significant bleeding and the presence of white spots on the larger hemorrhoid, it's essential to weigh the pros and cons of traditional surgery versus laser treatments. Let's break down your concerns and provide some clarity on the matter.
1. Understanding the Conditions: Hemorrhoids can be classified into different grades, with Grade II typically being characterized by prolapse that reduces spontaneously. The presence of white spots on your larger hemorrhoid raises the concern for potential complications, including the need for biopsy to rule out any malignancy. This is why your doctors are recommending surgical intervention.
2. Surgical Options:
- Traditional Surgery: This method involves excising the hemorrhoids and is often performed under general or local anesthesia. The advantage of traditional surgery is that it allows for thorough removal and examination of the tissue, which is crucial given your symptoms. However, it can lead to more postoperative pain and a longer recovery period.
- Laser Surgery: Laser treatment is considered a minimally invasive option. It typically results in less bleeding and pain, and the recovery time is often shorter. However, the effectiveness of laser treatment can vary based on the specific type of laser used and the skill of the surgeon. It's important to note that while some lasers may be less commonly used today, others, like the CO2 laser, are still effective for certain procedures.
3. Decision Making:
- Removing Only the Larger Hemorrhoid: If you choose to remove only the larger hemorrhoid, there is a possibility that the smaller ones could worsen over time. They may require treatment in the future, either through banding or surgery. However, leaving them in place may reduce immediate surgical trauma and complications.
- Removing All Hemorrhoids: On the other hand, opting for a complete excision of all hemorrhoids may prevent future issues and the need for additional surgeries. This approach can be more definitive but comes with the risk of increased pain and longer recovery.
4. Insurance and Cost Considerations: Since your insurance covers certain procedures, it's wise to confirm with your provider what is included in your coverage. This can influence your decision significantly, especially if the costs of traditional surgery versus laser treatment differ.
5. Postoperative Care and Risks: Regardless of the method chosen, there are risks associated with any surgical procedure, including infection, bleeding, and complications related to anesthesia. Discussing these risks with your surgeon is crucial. They can provide insights into the likelihood of complications based on your specific health profile.
6. Consultation with Your Surgeon: Given the complexity of your case, it would be beneficial to have a detailed discussion with your surgeon about the potential outcomes of each option. Ask about the specific types of lasers they use, their experience with them, and the expected recovery times for both traditional and laser surgeries.
In conclusion, both traditional and laser treatments have their merits and drawbacks. The decision should be based on a combination of your specific medical condition, the recommendations of your healthcare providers, and your personal preferences regarding recovery and potential risks. Make sure to ask all your questions during your consultations to ensure you feel comfortable and informed about your choice.
