Autonomic nervous system dysfunction
Hello Doctor, I am Xiao Chen, who inquired about autonomic nervous system disorders last week.
A couple of days ago, I returned to the psychosomatic clinic where I had previously been treated, and I also visited the gastroenterology department for symptoms of gastroesophageal reflux.
The psychiatrist prescribed Clonazepam 50mg, to be taken after dinner.
That night, after taking the medication, I had no issues and fell asleep around 1 AM without difficulty.
However, I woke up around 6 AM feeling nauseous, and after resting a bit, I fell asleep again, only to wake up around 11 AM feeling dizzy.
Since the day after starting the medication until today, I have experienced intermittent body tremors, twitching, sweating in my hands and feet, vivid dreams while falling asleep, sensitivity to cold in the air-conditioned room, and waking up around 6 or 7 AM feeling nauseous (with disrupted sleep).
Additionally, I have been experiencing intermittent chest pain, which I am unsure if it is caused by gastroesophageal reflux.
I am a bit worried that my body is not yet accustomed to taking Clonazepam after taking one pill the first night (the doctor mentioned that if I was concerned about side effects, I could start with half a pill).
Although I have taken this medication in the past, I still feel apprehensive.
I did not dare to take the medication for the past two days, yet I still have the symptoms mentioned above.
I would like to ask the doctor if the symptoms I have experienced in the past few days are side effects of the medication or if they are caused by autonomic nervous system dysfunction or gastroesophageal reflux.
Should I report these conditions to the psychiatrist? Thank you, Doctor!

Hello, Xiao Chen: If you have been off the medication for two days and still experience discomfort, it is more likely related to your underlying condition rather than a side effect of the medication.
It is recommended to follow your physician's advice and try taking half a tablet to see how you feel.
Additionally, it is advisable to report these symptoms to your doctor during your next visit.

Hello, 小陳. Thank you for reaching out with your concerns regarding your autonomic nervous system disorder and the medications you are currently taking. It’s understandable to feel anxious about the side effects of medication, especially when dealing with complex symptoms like those associated with autonomic nervous system disorders, anxiety, and gastrointestinal issues.
From your description, it seems you are experiencing a range of symptoms after starting the medication Clonazepam (克憂寧), which is a benzodiazepine used to treat anxiety and panic disorders. Benzodiazepines can have a variety of side effects, especially when you first start taking them or if the dosage is adjusted. Common side effects include drowsiness, dizziness, fatigue, and gastrointestinal disturbances such as nausea. The symptoms you mentioned—shaking, sweating, vivid dreams, and waking up feeling nauseous—could indeed be related to the medication, especially if you have recently started or increased the dosage.
However, it’s also important to consider that your symptoms may be exacerbated by your underlying conditions, such as autonomic nervous system dysfunction and gastroesophageal reflux disease (GERD). The autonomic nervous system regulates many involuntary bodily functions, including heart rate, digestion, and respiratory rate. When it is out of balance, it can lead to symptoms like those you are experiencing. For instance, anxiety can heighten the perception of bodily sensations, leading to feelings of shakiness or discomfort.
Regarding your concerns about the side effects of Clonazepam, it is crucial to communicate openly with your healthcare provider. If you are experiencing significant discomfort or if your symptoms worsen, it is advisable to report these changes to your psychiatrist or the prescribing physician. They may suggest adjusting your dosage or switching to a different medication that might be better tolerated. It’s also worth noting that some individuals may experience a period of adjustment when starting a new medication, during which side effects may diminish as your body acclimates.
In addition to medication, managing autonomic nervous system disorders often involves lifestyle modifications. Techniques such as mindfulness, cognitive-behavioral therapy (CBT), and relaxation exercises can be beneficial in reducing anxiety and improving overall well-being. Regular physical activity, a balanced diet, and adequate hydration can also support your nervous system and digestive health.
For your gastrointestinal symptoms, such as chest pain and nausea, it may be helpful to follow up with your gastroenterologist. They can provide guidance on managing GERD, which may include dietary changes, lifestyle modifications, or medications to reduce stomach acid.
In summary, while it is possible that your symptoms are related to the medication, they could also stem from your underlying conditions. It is essential to maintain open communication with your healthcare providers, who can help you navigate these challenges and adjust your treatment plan as necessary. Remember, you are not alone in this journey, and there are effective strategies and treatments available to help you manage your symptoms and improve your quality of life. Please take care, and don’t hesitate to reach out to your healthcare team for support.
