Right-sided flank pain?
Hello doctor, my question is that after drinking a lot of water at night, I experience pain on the right side of my lower back in the morning.
I'm not sure if it's due to pressure from holding in urine, but the pain disappears after I use the restroom.
I've tested this and found that the pain only occurs in the morning after holding in urine at night.
This phenomenon has been happening for a while now, and I haven't experienced it before.

Hello: Your condition should be evaluated by the urology department at the hospital to check for any abnormalities in the right kidney during urination.
If the tests are normal, we can explore other possible causes.
Wishing you good health.

Understanding right-sided lower back pain can be quite complex, as it may stem from various causes, including muscular, skeletal, or even visceral issues. Based on your description, it seems that your pain is correlated with your bladder fullness, which suggests that the discomfort might be related to pressure on the surrounding structures in your lower back when your bladder is distended.
When you drink a lot of water before bed, your bladder fills up overnight. If you hold it in until morning, the increased pressure from the full bladder can lead to discomfort in the lower back, particularly on the right side. This is because the bladder is located in the pelvic cavity, and as it expands, it can push against nearby organs and structures, including the lower back muscles and nerves. Once you relieve yourself, the pressure is alleviated, and the pain subsides, which supports the idea that your symptoms are related to bladder fullness rather than a more serious underlying condition.
However, it is important to consider other potential causes of right-sided lower back pain. These can include:
1. Muscle Strain: If you have recently engaged in activities that strain your back muscles, such as heavy lifting or awkward movements, this could lead to localized pain. Muscle strains can be exacerbated by certain positions, such as sitting or standing for prolonged periods.
2. Nerve Compression: Conditions like a herniated disc or spinal stenosis can lead to nerve compression, which may cause pain that radiates to one side of the lower back. This type of pain is often accompanied by other symptoms, such as numbness or tingling in the legs.
3. Kidney Issues: Right-sided lower back pain can sometimes be related to kidney problems, such as a kidney stone or infection. These conditions may cause pain that is more pronounced when pressure is applied to the area or when certain movements are made.
4. Visceral Pain: As mentioned, pain can also arise from internal organs. Conditions affecting the liver, gallbladder, or intestines can sometimes present as lower back pain, particularly if there is inflammation or other issues affecting these organs.
Given that this phenomenon has been occurring for some time, it would be prudent to consult with a healthcare professional. They may recommend imaging studies, such as an X-ray or MRI, to rule out any structural issues in the spine or surrounding areas. Additionally, a thorough physical examination can help determine if there are any muscular or postural issues contributing to your pain.
In the meantime, here are some strategies that may help alleviate your discomfort:
- Hydration Management: Consider adjusting your fluid intake before bedtime to minimize the need to hold urine overnight. This may help reduce the pressure on your lower back in the morning.
- Posture Awareness: Pay attention to your posture when sitting or standing. Maintaining a neutral spine position can help reduce strain on your back muscles.
- Gentle Stretching: Incorporating gentle stretching exercises for your lower back and hips can help relieve tension and improve flexibility.
- Heat Therapy: Applying a warm compress to the lower back may help relax tight muscles and alleviate discomfort.
- Physical Therapy: If the pain persists, a physical therapist can provide tailored exercises and treatments to strengthen your back and improve your posture.
In conclusion, while your symptoms appear to be related to bladder pressure, it is essential to monitor the situation and seek medical advice if the pain continues or worsens. Understanding the underlying cause of your right-sided lower back pain is crucial for effective management and prevention of future episodes.
