Gum swelling and pain?

Hello, doctor.
I have pain in both the upper and lower gums on the left side, and I also feel pain while chewing.
The pain worsens in the evening, but it improves after a night's sleep.
I also experience pain when pressing on the left tonsil.
My lower left wisdom tooth has only partially erupted, about three-quarters of the way.
Could the pain be related to the wisdom tooth? I had a similar issue before, but the doctor said there was no problem with my teeth.
My family has a history of periodontal disease.
Could this be a sign of periodontal disease?

Xi, 10~19 year old female. Ask Date: 2021/08/13

Dr. Zeng Guanghui reply Dentistry

It may be discomfort caused by gum inflammation or inflammation of the soft tissue surrounding the wisdom tooth crown.
It is recommended that if there is persistent pain, you should consult a dentist for further examination.

Gum pain can be a complex issue, especially when it involves wisdom teeth. Based on your description, it seems that your left side gums are experiencing discomfort, particularly during chewing, and the pain intensifies at night but subsides after sleep. Additionally, you mentioned that pressing on your left tonsil also causes pain, and your lower left wisdom tooth is partially erupted.

To address your concerns, let's break down the potential causes of your gum pain:
1. Wisdom Tooth Eruption: Since your lower left wisdom tooth is only partially erupted, it can create a few issues. Partially erupted teeth can lead to food trapping, which can cause inflammation and infection in the surrounding gum tissue, known as pericoronitis. This condition often results in localized pain, swelling, and sometimes even systemic symptoms like fever. The pain you feel while chewing could be a direct result of this inflammation.

2. Gum Disease: Given your family history of periodontal disease, it is essential to consider that your symptoms could be indicative of early gum disease, or gingivitis. Symptoms of gingivitis include swollen, tender gums that may bleed during brushing or flossing. If left untreated, gingivitis can progress to periodontitis, which can lead to more severe gum issues and tooth loss.

3. Referred Pain: The pain you feel when pressing on your left tonsil could be referred pain. This means that the discomfort in your gums or wisdom tooth could be causing pain sensations that radiate to nearby areas, including the throat and tonsils. This phenomenon is not uncommon, as the nerves in the head and neck are interconnected.

4. Other Dental Issues: It is also possible that there are other dental issues at play, such as cavities in adjacent teeth or issues with the alignment of your bite, which could be contributing to your discomfort.

Recommendations:
- Dental Evaluation: It is crucial to see a dentist for a thorough examination. They may take X-rays to assess the position of your wisdom tooth and check for any signs of infection or gum disease. If your dentist suspects that your wisdom tooth is causing the pain, they may recommend extraction, especially if it is impacted or causing recurrent issues.

- Oral Hygiene: Maintaining excellent oral hygiene is vital, especially with a partially erupted wisdom tooth. Make sure to brush and floss regularly, paying special attention to the area around the wisdom tooth to prevent food accumulation and plaque buildup.

- Monitor Symptoms: Keep track of your symptoms. If the pain worsens, or if you notice swelling, fever, or difficulty opening your mouth, seek dental care immediately, as these could be signs of a more serious infection.

- Consider Family History: Given your family history of periodontal disease, it may be beneficial to discuss preventive measures with your dentist, such as more frequent cleanings or specific oral care products designed to combat gum disease.

In summary, while your wisdom tooth could be a contributing factor to your gum pain, it is essential to have a comprehensive dental evaluation to rule out other potential causes and to determine the best course of action. Early intervention can prevent more severe complications and help maintain your oral health.
