Prepuce issues
Hello Doctor, I arrived in the United States about four days ago to prepare for college.
In the past two days, I have noticed that my foreskin is dry and cracked, and the glans is peeling.
Currently, I do not see any signs of redness or swelling, but one day I accidentally used body wash to clean the glans.
However, I have a habit of using soap to gently wash the glans in Taiwan, and I have never had any issues before.
Could this be due to the dry weather? What should I do to resolve this issue, or what medication should I buy (that is available at a pharmacy in the United States)?

Dr. Wu Yuxin reply Dermatology
It may be related to the local climate and humidity; you could try using a moisturizer.

More Info
Dealing with dryness and peeling of the foreskin can be a concerning issue, especially when you are in a new environment. Based on your description, it seems that you have experienced some irritation, possibly due to a combination of factors such as using body wash, changes in climate, or even just the natural sensitivity of the area. Here’s a comprehensive approach to understanding and managing your symptoms.
Possible Causes
1. Irritation from Products: Using body wash or soap that is not specifically designed for sensitive areas can lead to irritation. Many soaps contain fragrances and other chemicals that can disrupt the natural balance of the skin, leading to dryness and peeling.
2. Environmental Factors: Moving to a new location, especially if the climate is drier than what you are used to, can also contribute to skin dryness. Changes in humidity and temperature can affect your skin's moisture levels.
3. Hygiene Practices: While it’s important to maintain hygiene, over-washing or using harsh soaps can strip the skin of its natural oils, leading to dryness. It’s essential to find a balance.
4. Medical Conditions: Conditions such as balanitis (inflammation of the glans) or other dermatological issues can also cause similar symptoms. However, since you mentioned no redness or swelling, this may be less likely.
Recommended Solutions
1. Gentle Cleansing: Switch to a mild, unscented soap or a cleanser specifically formulated for sensitive skin. Look for products that are hypoallergenic and free from harsh chemicals. Brands like Cetaphil or Dove have options that are gentle on the skin.
2. Moisturizing: After washing, apply a fragrance-free moisturizer to the area. Products containing aloe vera or vitamin E can be soothing and help restore moisture. Avoid petroleum jelly or heavy ointments, as they may trap moisture but can also lead to irritation if used excessively.
3. Avoid Irritants: Refrain from using body wash or any products that contain alcohol, fragrances, or dyes on the genital area. Stick to water for cleansing, and limit the use of any soaps to the outer areas.
4. Hydration: Ensure you are drinking enough water to keep your body hydrated, which can also help your skin maintain moisture.
5. Over-the-Counter Treatments: If the dryness persists, you might consider over-the-counter hydrocortisone cream (1%) to reduce any inflammation. However, use it sparingly and not for extended periods. Always consult a pharmacist or healthcare provider before starting any new medication.
6. Consult a Healthcare Professional: If symptoms do not improve within a few days or if you notice any additional symptoms such as redness, swelling, or pain, it is advisable to consult a healthcare professional. They can provide a proper diagnosis and treatment plan.
When to Seek Help
If you experience any of the following, seek medical attention:
- Persistent dryness or peeling that does not improve with home care.
- Development of redness, swelling, or pain.
- Any discharge or unusual odor.
- If you have any underlying health conditions that may affect skin health.
Conclusion
In summary, the dryness and peeling of the foreskin you are experiencing could be due to a variety of factors, including irritation from products, environmental changes, or hygiene practices. By switching to gentle cleansing methods, moisturizing regularly, and avoiding irritants, you can help alleviate your symptoms. If the issue persists or worsens, do not hesitate to seek medical advice. Remember, taking care of your skin is essential, especially in a new environment.
