Bleeding due to the opening of the incision after circumcision stitches have come undone?
Hello Doctor, I am currently about three weeks post circumcision, and the stitches have gradually come loose.
One of the stitches has fallen out, and there seems to be a split in the wound, which is oozing blood.
I can see a noticeable gap and some white tissue.
Do I need to return for a follow-up to have it re-stitched, or is it sufficient to continue applying medication and disinfecting the area? Thank you for taking the time to respond.

Dr. Li Minghui reply Urology
Hello: Most small wounds do not become inflamed and will heal on their own if kept clean and dry.
Follow-up visits after outpatient surgery are necessary.
Wishing you good health.

More Info
After circumcision, it is common to have concerns about the healing process and potential complications. Based on your description, it seems that you are experiencing some issues with the surgical site, particularly with a visible opening and bleeding. Here’s a detailed overview of what to consider regarding post-circumcision complications and when to seek medical attention.
Normal Healing Process
Typically, after a circumcision, the healing process can take anywhere from a few weeks to a couple of months. During this time, it is normal for the stitches to dissolve and for the area to appear red or slightly swollen. Discomfort and mild bleeding can also occur, especially if the area is irritated or if there is friction from clothing.
Signs of Complications
However, certain signs may indicate complications that require medical attention. These include:
1. Excessive Bleeding: If you notice that the bleeding is persistent or soaking through the bandage, this could be a sign of a problem.
2. Infection: Signs of infection include increased redness, swelling, warmth, or pus-like discharge from the wound. Fever may also accompany these symptoms.
3. Wound Dehiscence: This refers to the reopening of the wound. If you see a significant gap in the skin or if the edges of the wound are not approximating, this could indicate that the wound is not healing properly.
4. Severe Pain: While some discomfort is expected, severe pain that is not relieved by over-the-counter pain medications may warrant further evaluation.
5. Visible Tissue: If you can see white or yellow tissue (which could be granulation tissue or fat) and it is not healing, this is a concern.
Your Situation
In your case, since you mentioned that there is a visible crack in the wound with some bleeding and exposure of tissue, it is advisable to seek medical attention. Here’s why:
- Potential for Infection: An open wound can easily become infected, especially if it is exposed to bacteria.
- Need for Re-evaluation: A healthcare provider can assess the wound properly, determine if it needs to be re-stitched, and provide appropriate treatment to promote healing.
- Proper Care Instructions: A medical professional can give you tailored advice on how to care for the wound, including whether to continue with topical medications or if other treatments are necessary.
What to Do Next
1. Contact Your Surgeon or Urologist: It’s best to reach out to the healthcare provider who performed the circumcision. They will be familiar with your specific case and can provide the most accurate advice.
2. Avoid Irritation: Until you can be seen, try to keep the area clean and dry. Avoid any activities that could irritate the site, such as vigorous exercise or sexual activity.
3. Monitor Symptoms: Keep an eye on any changes in the wound, including increased redness, swelling, or discharge. If you develop a fever or if the bleeding worsens, seek immediate medical attention.
Conclusion
In summary, while some discomfort and minor issues can be expected after circumcision, the symptoms you are describing—particularly the visible crack and bleeding—suggest that you should seek medical attention. It’s better to err on the side of caution to ensure proper healing and to prevent complications. Your healthcare provider will be able to assess the situation and recommend the best course of action.
