Ear problems
Hello, doctor.
Recently, while cleaning my ears, I felt some water enter.
I used a piece of tissue to clean my ear, and I think I may have accidentally inserted it too deeply.
Although I didn't apply much force, I felt a slight poke to my eardrum, which caused a brief pain that quickly subsided.
I'm not sure if I actually touched my eardrum.
This morning, when I pressed on my ear, I noticed a rustling sound that only occurs when I apply pressure; there’s no sensation when I don’t press.
I would like to know what this symptom might be.
I have been worried that I might have ruptured my eardrum, but I don't experience any other symptoms like tinnitus or pain.

When it comes to ear issues, particularly those related to potential eardrum injury, it's essential to understand the anatomy of the ear and the symptoms that may arise from trauma or irritation. The eardrum, or tympanic membrane, is a thin membrane that separates the outer ear from the middle ear. It plays a crucial role in hearing by vibrating in response to sound waves. If you suspect that you may have injured your eardrum, there are several symptoms and considerations to keep in mind.
Based on your description, you experienced a sensation of pain when you attempted to clean your ear, followed by a "crackling" or "rustling" sound when you press on your ear. This could indicate a few possibilities:
1. Eustachian Tube Dysfunction: The Eustachian tube connects the middle ear to the back of the throat and helps equalize pressure. If you have water in your ear or if you’ve created a vacuum by pressing on your ear, you might hear sounds like crackling or popping. This is often harmless and can resolve on its own.
2. Earwax Impaction: Sometimes, cleaning the ear can push earwax deeper into the ear canal, leading to a blockage. This can cause a sensation of fullness, pressure, or sounds when you manipulate your ear. If this is the case, it may require professional cleaning by a healthcare provider.
3. Tympanic Membrane Injury: While you mentioned that you did not experience significant pain or other symptoms like hearing loss or tinnitus (ringing in the ears), it is still possible to have a minor injury to the eardrum. Symptoms of a perforated eardrum can include sudden sharp pain, drainage from the ear, hearing loss, or a feeling of fullness. If the eardrum is intact, you may not experience these symptoms.
4. Middle Ear Issues: Sometimes, issues in the middle ear, such as fluid accumulation or infection, can cause similar sensations. If you feel pressure or hear sounds when manipulating your ear, it could be related to changes in pressure within the middle ear.
Given that you do not have significant pain or other alarming symptoms, it may not be an emergency. However, it is essential to monitor your symptoms. If the crackling sound persists, or if you develop additional symptoms such as pain, hearing loss, or discharge from the ear, you should seek medical attention. An ENT (ear, nose, and throat) specialist can perform an examination, potentially using an otoscope to visualize the eardrum and determine if there is any injury or other underlying issue.
In the meantime, avoid inserting anything into your ear canal, including cotton swabs or fingers, as this can exacerbate any potential injury or cause further blockage. If you suspect water is trapped in your ear, you can try tilting your head to the side or using gravity to help drain it out.
In summary, while your symptoms may not indicate a severe issue, it is crucial to remain vigilant. If you experience any changes or worsening of your symptoms, do not hesitate to consult a healthcare professional for a thorough evaluation and appropriate management.
