Prostatitis
Doctor, I would like to ask you a few questions.
I had prostatitis during the first semester of my first year in high school.
At that time, I went to a nearby community hospital for examination and medication.
After about three weeks, my symptoms significantly improved, and issues like frequent urination and difficulty urinating were greatly alleviated, so my attending physician discontinued the medication.
After some time, I noticed that the symptoms still occasionally reappeared.
For example, sometimes after drinking a large cup of bubble tea, I find myself needing to use the restroom quite frequently, about every ten minutes or so, while at other times it seems normal.
The difficulty in urination also occurs occasionally, where I need some time to urinate, but it is only sometimes.
However, there is another issue that concerns me.
Sometimes when I sit on a chair, I feel a strange sensation in the perineum or deep within the penis, which is hard to describe, somewhat like soreness.
Occasionally, due to friction from clothing, I experience slight penile engorgement, and when I retract the penis, I have a similar sensation.
Additionally, once while showering, when I was cleaning around the anal area, I pressed on the perineum near the area just below the scrotum, and I felt a very noticeable soreness.
I want to ask the doctor what this situation is.
Could it be a recurrence of my old condition? Surely it can't be something serious like cancer, right? Please help clarify this for me.
Thanks!

Hello: Your symptoms may not necessarily indicate a recurrence, but it is essential to pay attention to your lifestyle, including diet, hydration, and activity levels.
If you are engaging in sexual activity, practicing safe sex is important.
As for the likelihood of other diseases, it is relatively low.
However, to confirm whether there are any issues, it is necessary to undergo an examination at a larger hospital.
It is best to seek evaluation when symptoms are present, as this increases the chances of identifying any problems.
Wishing you good health.

More Info
Prostatitis, particularly chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS), can be a challenging condition to manage, as it often presents with a variety of symptoms that can significantly impact a person's quality of life. Your experience of recurrent urinary symptoms, such as frequency and difficulty urinating, along with discomfort in the pelvic region, is not uncommon among those with prostatitis.
Symptoms and Concerns
The symptoms you describe, including frequent urination, urgency, and pelvic discomfort, are characteristic of prostatitis. The intermittent nature of your symptoms, where they improve and then recur, is also typical of chronic prostatitis. The sensations you experience, such as the peculiar feeling in the perineum or penis, can be attributed to pelvic floor muscle tension or irritation, which is common in this condition.
It's important to note that while prostatitis can be uncomfortable and distressing, it is not typically associated with serious complications like cancer. The symptoms you are experiencing do not suggest an immediate risk of malignancy, especially if you have been evaluated by a healthcare provider and have not shown any alarming signs. However, persistent or worsening symptoms should always be evaluated by a medical professional to rule out other conditions.
Treatment Options
Given that you have previously responded to antibiotic treatment, it is essential to consider that chronic prostatitis may not always be due to bacterial infection. In many cases, the condition is non-bacterial, and treatment may require a multifaceted approach. Here are some strategies that may help:
1. Medication: While antibiotics may have been effective initially, their long-term use is not always beneficial in non-bacterial prostatitis. Other medications, such as alpha-blockers (which help relax the bladder neck and prostate), anti-inflammatory medications, or muscle relaxants, may provide relief. Discussing these options with your healthcare provider could be beneficial.
2. Physical Therapy: Pelvic floor physical therapy can be particularly effective for managing symptoms related to pelvic muscle tension. A physical therapist specializing in pelvic health can help you learn exercises to relax and strengthen the pelvic floor muscles.
3. Lifestyle Modifications: Maintaining a healthy lifestyle can significantly impact your symptoms. Staying hydrated, avoiding irritants such as caffeine and alcohol, and practicing stress management techniques can be beneficial. Regular exercise and pelvic floor exercises (like Kegel exercises) may also help alleviate symptoms.
4. Heat Therapy: Many patients find relief through heat therapy, such as warm baths or heating pads applied to the pelvic area. This can help relax the muscles and reduce discomfort.
5. Alternative Therapies: Some patients report benefits from acupuncture or biofeedback therapy. While these methods may not be universally effective, they are worth discussing with your healthcare provider.
6. Regular Follow-ups: Since your symptoms have recurred, it is crucial to maintain regular follow-ups with your healthcare provider. They can monitor your condition and adjust your treatment plan as necessary.
Conclusion
In summary, while your symptoms can be distressing, they are not uncommon in the context of prostatitis. It is unlikely that you are facing a severe underlying condition like cancer, especially if you have been evaluated by a physician. However, persistent symptoms warrant ongoing medical attention. A combination of medication, physical therapy, lifestyle changes, and possibly alternative therapies may provide relief. Always consult with your healthcare provider before making any changes to your treatment plan to ensure that it is safe and appropriate for your specific situation.
