Persistent High-Pitched Tinnitus: Causes and Solutions - Otolaryngology

Suddenly, after one night, I began to hear a continuous high-pitched ringing in both ears?

Hello Doctor, my father suddenly heard a loud noise one night (only he heard it, and he suspects it was a sound in his head) and woke up.
Since then, he has been experiencing persistent high-pitched ringing in both ears, which has not ceased to this day.
He went to the hospital for examination, and they found no brain tumor, normal hearing pressure, and no middle ear infection; all tests have shown no abnormalities.
Currently, aside from the continuous high-frequency sound and sleep disturbances (the tinnitus allows him to sleep only 3 to 4 hours a day), he is very sensitive to sound, but he does not experience headaches or dizziness.
His blood pressure is within the normal range, and his overall health is good.
The only related issue is nasal allergies, but after examination, the doctor said it is not significantly related.
During the examination, external factors were mentioned; there is a woodworking circular saw operating across the street, which occasionally produces sharp noise disturbances during the day.
Before the onset of tinnitus, my father had difficulty falling asleep (light sleep) but could still sleep 6 to 8 hours at night.
In previous consultations, the doctors could not identify the cause and suggested it might be age-related degeneration.
Therefore, I would like to ask the doctor if my father can still visit a specialist for this issue? If it is indeed degeneration, what exercises, foods, or supplements would you recommend to prevent further deterioration? Or do you have any other suggestions? Thank you for your help, Doctor! Wishing you a lifetime of peace and joy! Thank you very much!

Dr. Li Jiaxuan reply Otolaryngology

It is recommended to visit an otolaryngology clinic for treatment and to avoid vigorous exercise or consuming heavily flavored foods.
However, it is important to maintain a regular sleep schedule.
If the condition is simply due to degeneration and there are no contraindications for taking vitamins, it is advisable to supplement with B vitamins.

More Info

Persistent high-pitched tinnitus, often described as a ringing or buzzing sound in the ears, can be a distressing condition, particularly when it disrupts sleep and daily life. Your father's experience of sudden onset tinnitus following a loud noise is not uncommon, and while it can be challenging to pinpoint the exact cause, there are several potential factors and solutions to consider.

Causes of Tinnitus
1. Noise Exposure: Exposure to loud noises, such as the circular saw operating across the street, can lead to temporary or permanent changes in hearing, resulting in tinnitus. Even if your father did not experience immediate hearing loss, the auditory system can be sensitive to such disturbances.

2. Age-Related Hearing Loss: Presbycusis, or age-related hearing loss, is a common cause of tinnitus in older adults. The inner ear structures may deteriorate over time, leading to changes in how sound is processed.

3. Eustachian Tube Dysfunction: Although your father's ear pressure was reported as normal, issues with the Eustachian tube can sometimes contribute to tinnitus.

4. Stress and Anxiety: Psychological factors can exacerbate tinnitus. The stress of dealing with persistent noise can create a cycle where anxiety increases the perception of tinnitus.

5. Allergies and Sinus Issues: Allergies can lead to congestion and pressure changes in the ear, which may contribute to tinnitus symptoms.

Management and Solutions
1. Consultation with Specialists: Since initial evaluations have not revealed any significant issues, it may be beneficial for your father to consult an otolaryngologist (ENT specialist) or an audiologist specializing in tinnitus management. They can provide further assessments and potential treatment options.

2. Sound Therapy: Using background noise or white noise machines can help mask the tinnitus sounds, making them less noticeable, especially during sleep.

3. Cognitive Behavioral Therapy (CBT): This therapeutic approach can help individuals manage the emotional response to tinnitus, reducing anxiety and improving coping strategies.

4. Hearing Aids: If hearing loss is present, hearing aids can amplify external sounds, which may help mask the tinnitus.

5. Lifestyle Changes: Encouraging your father to adopt a healthy lifestyle can be beneficial. Regular exercise, a balanced diet rich in fruits, vegetables, and omega-3 fatty acids, and staying hydrated can support overall health and potentially mitigate tinnitus symptoms.

6. Avoiding Triggers: Identifying and avoiding potential triggers, such as loud noises, caffeine, and nicotine, may help reduce the severity of tinnitus.

7. Supplements: Some studies suggest that certain supplements, such as ginkgo biloba, may help with tinnitus, although evidence is mixed. It's essential to consult with a healthcare provider before starting any new supplements.

Conclusion
While persistent high-pitched tinnitus can be frustrating and disruptive, there are various avenues for management and relief. It is crucial for your father to continue seeking medical advice and exploring different treatment options. Encouraging him to maintain a healthy lifestyle and manage stress can also play a significant role in improving his quality of life. If symptoms persist or worsen, further evaluations may be necessary to rule out any underlying conditions. Your father's well-being is paramount, and with the right approach, he can find ways to cope with and potentially reduce the impact of tinnitus on his life.
