I feel a swollen and very painful area around the anus?

Hello, doctor.
Two days ago, I suddenly started experiencing itching around my anus, and I can also feel some slight swelling.
When I have a bowel movement, the area comes into contact with the stool, causing significant pain.
Currently, there is no blood in my stool, and I haven't noticed any blood on the toilet paper.
By yesterday, the pain has become more pronounced, and I can feel it clearly when sitting or lying down.
Standing or lying face down alleviates the pain significantly.
A few days prior to this, I accidentally wiped too hard after using the restroom, which was quite painful at the time.
I would like to know if this is a case of anal fissure or hemorrhoids.
Are there any treatment options available?

What issues require a physician's personal examination for diagnosis? It is impossible to make a definitive diagnosis based solely on a written description, and online consultations cannot replace actual medical evaluations.
Based on your description, it seems more like an anal fissure or thrombosed external hemorrhoid, but anal warts or an anal abscess are also possibilities.
Please seek a physician's examination and treatment as soon as possible to avoid delaying your condition.

More Info

Painful swelling near the anus can be caused by a variety of conditions, with the most common being hemorrhoids, anal fissures, or even infections. Given your symptoms of itching, swelling, and pain during bowel movements, it is essential to consider these possibilities.

1. Hemorrhoids: Hemorrhoids are swollen veins in the lower rectum and anus, which can cause discomfort, itching, and pain, especially during bowel movements. They can be internal (inside the rectum) or external (under the skin around the anus). The fact that you have experienced pain after wiping too hard may suggest the presence of external hemorrhoids, which can become inflamed and painful.

2. Anal Fissures: An anal fissure is a small tear in the lining of the anus, often caused by passing hard stools or straining during bowel movements. This can lead to sharp pain during and after bowel movements, as well as itching and discomfort. The history of wiping too hard could have contributed to this condition.

3. Infections or Abscesses: In some cases, an infection or abscess near the anus can cause swelling and pain. This is less common but should be considered if symptoms worsen or if you develop fever or other systemic symptoms.

4. Skin Conditions: Conditions such as dermatitis or fungal infections can also cause itching and discomfort around the anal area.

Remedies and Management
Here are some steps you can take to manage your symptoms:
- Warm Sitz Baths: Soaking in warm water for 10-15 minutes several times a day can help reduce pain and swelling. This can also promote healing.

- Over-the-Counter Treatments: You can use topical treatments such as hydrocortisone cream or witch hazel pads to relieve itching and inflammation. There are also specific creams available for hemorrhoids that can provide relief.

- Dietary Changes: Ensure you are consuming a high-fiber diet to prevent constipation and straining during bowel movements. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Staying hydrated is also crucial.

- Gentle Wiping: After bowel movements, use moistened wipes instead of dry toilet paper to avoid further irritation. Be gentle when cleaning the area.

- Pain Relief: Over-the-counter pain relievers such as acetaminophen or ibuprofen can help manage pain.

- Avoid Straining: When using the bathroom, try to relax and avoid straining, as this can exacerbate the problem.

When to Seek Medical Attention
If your symptoms do not improve with home treatment, or if you experience severe pain, bleeding, or signs of infection (such as fever or pus), it is important to seek medical attention. A healthcare provider can perform a physical examination and may recommend further treatments, such as prescription medications or procedures if necessary.

In conclusion, while your symptoms may suggest hemorrhoids or an anal fissure, proper evaluation by a healthcare professional is essential for an accurate diagnosis and appropriate treatment. Taking care of your diet, practicing good hygiene, and using home remedies can help alleviate your discomfort in the meantime.
